A case report of invasive candidiasis

Abstract:A 71-year-old woman was admitted to Shengli Oil Field Central Hospital on September 2,2021,due to"recurrent abdominal pain for over 10 days".Initially,the patient presented with abdominal pain as the predominant symptom,later progressed to low back pain with restricted movement.The patient was in a forced lateral decubitus position.A approximately 20 cm long surgical scar from a previous cholecystectomy,which was visible in the right upper abdomen.Left abdominal tenderness,thoracolumbar kyphosis,and localized tenderness with rebound pain were present.After admission,comprehensive investigations were performed.On September 18,2021,the patient underwent a CT-guided thoracic vertebra biopsy.Culture of the aspirated lavage fluid showed Candida albicans.Following definitive diagnosis,antifungal therapy with fluconazole was initiated for 5 days without clinical improvement.Treatment was switched to micafungin for more than 2 months,followed by sequential oral fluconazole capsules.The patient's condition stabilized and she was discharged on January 29,2022 with continued fluconazole therapy.Regular follow-ups were maintained in the Infectious Diseases Clinic until fluconazole was discontinued in June 2023,post-treatment monitoring for 6 months showed no evidence of disease recurrence.
