Analysis of pathogen characteristics and influencing factors of abdominal fungal infection after anatomical resection for hepatocellular carcinoma

Abstract:Objective To observe the pathogen characteristics of abdominal fungal infection after anatomical resection in patients with hepatocellular carcinoma,and to analyze the related influencing factors.Methods Forty-six patients who de-veloped abdominal fungal infection within 30 days after anatomical hepatectomy in the Department of Hepatobiliary Surgery,First Affiliated Hospital of Air Force Military Medical University from August 2021 to June 2024 were retrospectively select-ed as the infection group.Ninety-two patients with hepatocellular carcinoma who underwent anatomical hepatectomy without fungal infection during the same period were selected as the control group in a 1∶2 ratio.Clinical data of the two groups were collected and compared.Univariate and logistic regression analyses were performed to analyze the pathogen characteris-tics of abdominal fungal infection and its influencing factors.Results A total of 48 fungal strains were isolated from patients in the infection group,of which Candida albicans accounted for 45.83%(22/48).Univariate analysis showed that the pro-portion of patients with age≥60 years,diabetes,liver function grade B,microvascular invasion,open surgery,drainage tube indwelling time>7 d,intestinal flora dysbiosis,and operation time were all higher in the infection group than those in the control group,while albumin level was lower than that in the control group(P<0.05).Logistic regression analysis showed that abdominal fungal infection after anatomical resection was independently associated with microvascular invasion,diabetes,intestinal flora dysbiosis,albumin,operation time,and drainage tube indwelling time(P<0.05).Conclusion Candida albicans is the common pathogen in abdominal fungal infection after anatomical resection for hepatocellular carcinoma.Diabetes,mi-crovascular invasion,operation time,drainage tube indwelling time,intestinal flora dysbiosis,and albumin are all influen-cing factors.Protective measures can be taken accordingly in clinical practice to reduce the incidence of postoperative abdomi-nal fungal infection.
