Clinical characteristics and significance of mixed Candida/bacterial bloodstream infections in patients with hematological diseases

Abstract:Objective Exploring the clinical characteristics and significance of mixed Candida/bacterial bloodstream infec-tions in patients with hematological diseases.Methods A retrospective study was conducted on 50 blood disease patients with Candida bloodstream infection admitted to our hospital from July 2018 to August 2023.The patients were divided into a mixed Candida/bacterial bloodstream infection group of 14 cases and a single Candida bloodstream infection group of 36 cases based on whether they had bacterial bloodstream infection.Analyze the influencing factors,strain types,and 30 day survival status of mixed Candida/bacterial bloodstream infections.Results Binary logistic regression analysis showed that organic injury,septic shock,hospitalization for more than 4 weeks,and positive PCT test were all influencing factors of mixed Candida/bacterial bloodstream infection(P<0.05).Among 50 patients with Candida infection,38 strains of Can-dida albicans(76.00%),6 strains of Candida tropicalis(12.00%),and 6 strains of other Candida(12.00%)were iden-tified;Among 14 patients with mixed Candida/bacterial bloodstream infections,12 strains(85.71%)were Gram-negative bacteria,including 5 strains(35.71%)of Klebsiella pneumoniae,4 strains(28.57%)of Oligotropha maltophila,2 strains(14.29%)of Pseudomonas aeruginosa,and 1 strain(7.14%)of the others;There were 2 cases(14.29%)of Gram-positive bacteria.There were 6 deaths within 30 days in the mixed Candida/bacterial bloodstream infection group,account-ing for 42.86%,and 5 deaths within 30 days in the single Candida bloodstream infection group,accounting for 13.89%.The mortality rate within 30 days of the mixed Candida/bacterial bloodstream infection group was higher than that of the single Candida bloodstream infection group(χ2=4.929,P<0.05).Kaplan Meier survival analysis showed that the overall survival of the single Candida bloodstream infection group was better than that of the mixed Candida/bacterial bloodstream infection group,and the differences were statistically significant(P<0.05).Conclusion The mixed Candida/bacterial bloodstream infection in patients with hematological diseases is mainly caused by Gram-negative bacteria,and the mortality rate significantly increases after 30 days.Factors affecting mixed Candida/bacterial bloodstream infection include organic in-jury,septic shock,hospitalization for more than 4 weeks,and positive PCT test.
