Qualitative Analysis of Target Management in Oncology Performance Evaluation under Actor-Network Theory Framework

Abstract:Objective To investigate the current status,challenges,and improvement strategies for implementing tumor diagnosis and treatment performance indicator target management in a district of Beijing.Methods A qualitative study was conducted using semi-structured interviews with 33 staff members from 1 1 medical institutions providing oncology services in the district.Interview data were coded through thematic analysis.Under the framework of Actor-Network Theory,implementation was analyzed across four dimensions:actor identification,translation,obligatory passage points,and network stability/conflicts.Results The District Tumor Quality Control Center organizes annual on-site supervision and inspection,quarterly self-inspections and random checks,and monthly monitoring indicator data collection,effectively promoted practitioners'voluntary compliance with guidelines and standards.These measures standardized and homogenized diagnosis and treatment practices across institutions while improving indicator performance.However,unmet target values were primarily attributed to tumor patients'individualized treatment needs and inconsistent acceptance of assessment indicators among physicians.Conclusion The standardized and homogeneous management requirements for tumor diagnosis and treatment stand in both contradiction and unity with the heterogeneity of cancer patients and the variations among hospitals,departments,and physicians.
