Predictive value of preoperative leukocyte-derived markers for microvascular invasion after interventional therapy for hepatocellular carcinoma

Abstract:Objective To evaluate the predictive value of preoperative leukocyte-derived indicators for microvascular invasion(MVI)after interventional surgery for liver cancer.Methods A total of 100 liver cancer patients who underwent interventional therapy and were admitted to Baoding Second Central Hospital from January 2022 to January 2025 were included.According to the postoperative pathological results,the patients were divided into the MVI positive group and the MVI negative group.The clinical data of the patients were collected,and the general data,imaging features and leukocyte-derived indicators of the two groups were compared.Logistic regression analysis was used to screen the influencing factors,and the ROC curve was used to evaluate the predictive efficacy.Results After clinical evaluation,among the 100 liver cancer patients who underwent interventional therapy included in this study,the detection rate of MVI after surgery was 32.00%(32/100).The results of univariate analysis showed that the peritumoral enhancement,unclear tumor boundary and the proportion of intratumoral necrosis in the MVI positive group were all higher than those in the MVI negative group(P<0.05),the neutrophil/lymphocyte ratio(NLR),platelet/lymphocyte ratio(PLR)and systemic immune inflammation index(SII)values were all higher than those in the MVI negative group(P<0.05).Logistic regression analysis showed that peritumoral enhancement(OR=1.582),unclear tumor boundary(OR=1.488),intratumoral necrosis(OR=1.597),NLR(OR=1.605),PLR(OR=1.492),and SII(OR=1.620)are risk factors affecting MVI after interventional surgery for liver cancer(P<0.05).The results of the ROC curve showed that the sensitivity and AUC of the combined preoperative leukocyte-derived indicators in predicting MVI after interventional surgery for liver cancer were 90.50%and 0.902 respectively,which were higher than 85.00%and 0.875 predicted by the combined traditional imaging features.Conclusions The predictive value of preoperative leukocyto-derived indicators for MVI after interventional surgery for liver cancer is higher than that of traditional imaging features,which can be used as non-invasive predictive tools to assist in the clinical identification of patients at high risk of MVI and provide a reference for individualized treatment decisions.
