Association of visceral obesity index with surgical efficacy,prognosis and early inflammatory bowel obstruction in colorectal cancer

Abstract:Objective To analyze the association of visceral obesity index(VAI)with the surgical efficacy,prognosis and early inflammatory bowel obstruction in colorectal cancer.Methods A total of 100 patients with colorectal cancer who received laparoscopic surgery in Beijing Hospital of Traditional Chinese Medicine Affiliated to Capital Medical University from January 2021 to December 2023 were selected.With the median VAI as the dividing point,patients were divided into high VAI group and low VAI group,and clinical data of the two groups were compared.In addition,the clinical efficacy,early inflammatory bowel obstruction and prognosis of the two groups were recorded and compared.ROC curve was used to analyze the value of VAI in predicting patients with early postoperative inflammatory bowel obstruction.Results There was no significant difference between high VAI group and low VAI group in gender,age,history of hypertension,history of diabetes,ECOG score,tumor location,TNM stage,degree of tumor differentiation,amount of blood loss,postoperative exhaust time,postoperative feeding time,length of hospital stay and other data(P>0.05).There were significant differences in body mass index(BMI),waist circumference,triglyceride,high density lipoprotein cholesterol,operation time and conversion rate to laparoscopic surgery between the two groups(P<0.05).The total effective rate of treatment in the high VAI group(78.00%)was slightly higher than that in low VAI group(72.00%)(P>0.05).The postoperative local recurrencerate(20.00%)anddistantmetastasis rate(16.00%)in high VAI group were slightly higher than those in low VAI group(16.00%and 14.00%)(P>0.05),while the incidence of early inflammatory bowel obstruction(28.00%)was significantly higher than that in low VAI group(10.00%)(P<0.05).ROC curve showed that the area under the curve of VAI was 0.860(0.787-0.933)(P<0.05),the sensitivity was 0.843,the specificity was 0.716,and the optimal cutoff value was 12.2,indicating that VAI had a high predictive value for patients with early postoperative inflammatory bowel obstruction.Conclusions When VAI is higher,it will increase the operation time and the risk of early postoperative inflammatory bowel obstruction in colorectal cancer patients,but it has no significant effect on the surgical efficacy and prognosis of patients.In addition,VAI has a high predictive value for early postoperative inflammatory bowel obstruction.
