Clinical influencing factors and inflammatory indices of radiatherapy-related oropharyngeal candidiasis in patients with head and neck squamous cell carcinoma

Abstract:Objective To analyze the risk factors of radiotherapy-related oropharyngeal candidiasis and the serum inflammatory indexes TNF-α,TGF-β1 and IL-6 in patients with head and neck malignant tumors,.Methods A total of 218 patients with head and neck tumors treated in the Eighth Affiliated Hospital of Guangxi Medical University from January 2018 to January 2020 with complete clinical data and relevent follow-up information were collected,and univariate analysis was carried out to screen the suspected risk factors for the incidence of oropharyngeal candidiasis,and the independent risk factors were analyzed by binary Logistic regression analysis.Randomly selected 30 patients with or without oral and pharyngeal candidiasis during radiotherapy to detect and analyze TNF-α,TGF-β1 and IL-6.Results All patients had oral mucositis of different degree,68.3%( 149/218 ) of grade 1~2 and 31.7%( 69/218 ) of grade 3.Oropharyngeal candidiasis occurred in 20.2%( 44/218) patients.Logistic regression analysis showed an increased incidence of oropharyngeal candidiasis in patients with oropharyngeal tumors,leukopenia,anemia,and grade 3 oral mucositis during radiotherapy.Serum levels of TNF-α,TGF-β1 and IL-6 in patients with oropharyngeal candidiasis were higher than those in patients without oropharyngeal candidiasis.Conclusions Patients with oropharyngeal cancer and grade 3 acute radiation oral mucositis are independent risk factors for oropharyngeal candidiasis.TNF-α,TGF-β1 and IL-6 are important inflammatory indicators for the occurrence of radiation oropharyngeal candidiasis.
