Multimodal prevention strategies for postoperative low deep vein thrombosis in patients with tumors and the impact on quality of life:A prospective cohort study

Abstract:Objective To explore the multimodal prevention strategies for postoperative low deep vein thrombosis (LDVT) in patients with tumors and the impact on quality of life.Methods A total of 216 postoperative cancer patients who were treated in Taicang Hospital of Traditional Chinese Medicine from January 2020 to January 2024 were selected as the study subjects.The subjects were randomly divided into a multimodal group and a control group,with 108 cases in each group.The multimodal group received multimodal prevention including drugs,mechanical preventions,physical activities,education and guidance,while the control group received conventional anticoagulation therapy.The incidence of LDVT,activated partial thromboplastin time (APTT),prothrombin time (PT),D-dimer (D-D),fibrinogen (FIB),deep vein blood flow velocity and quality of life were compared between the two groups.At the same time,the study subjects were divided into LDVT group and non-LDVT group,and the risk factors affecting the occurrence of LDVT were analyzed by Logistic regression to adjust potential confounding factors.Results Compared with the control group,the levels of APTT and PT in multimodal group were longer on the third day after operation ( P<0.05),and the levels of D-D and FIB were lower ( P<0.05) .Compared with the control group,bilateral deep vein blood flow velocity in multimodal group was higher at 3,5 and 7 days after operation (P<0.05).The incidence of postoperative LDVT in the multimodal group ( 4.63%) was significantly lower than that in the control group ( 20.37%) (x2=12.233,P<0.001) .Compared with the control group,after treatment,the scores of all dimensions of the SF-36 scale in the multimodal group were higher ( P<0.05) .Compared with non-LDVT group,the proportion of postoperative multimodal preventive measures and the levels of APTT and PT were lower in LDVT group ( P<0.05),and the levels of D-D and FIB were higher in LDVT group (P<0.05).Logistic regression analysis showed that APTT,PT,D-D and FIB levels were independent risk factors for postoperative LDVT in patients with malignant tumors ( P<0.05 ),and postoperative multimodal prevention strategies were independent protective factors for postoperative LDVT in patients with malignant tumors ( P<0.05 ) .ConclusionsPostoperative multimodal prevention strategy can effectively reduce the risk of postoperative LDVT in patients with malignant tumors and improve the quality of life,and is an independent protective factor affecting the occurrence of postoperative LDVT in patients with malignant tumors,which may be related to the fact that multimodal prevention strategy is more conducive to improving the postoperative blood hypercoagulability of patients.
