Comparison of the effects of concurrent and staged TURBT/PKRP surgery in patients with bladder cancer complicated with prostatic hyperplasia

Abstract:Objective To analyze the effects of simultaneous and staged transurethral resection of bladder tumor(TURBT)/transurethral plasmotomy of prostate(PKRP)on postoperative recovery in patients with bladder cancer complicated with prostatic hyperplasia.Methods A total of 98 patients with bladder cancer complicated with prostatic hyperplasia who were admitted to Shanxi Cancer Hospital from December 2020 to December 2023 and received TURBT/PKRP treatment.Fifty-two patients receiving concurrent TURBT and PKRP treatment were included in the concurrent group,and 46 patients receiving staged TURBT and PKRP treatment were included in the staged group.The curative effect,prostatic function index(maximum urine flow rate(Qmax),residual urine volume(RU),international prostatic symptom(IPSS)score,maximum urethral closure pressure,maximum urethral pressure),and prognosis(bladder cancer recurrence,prostatic fossa implantation metastasis)were compared between the two groups.Results The curative effect of the concurrent group was better than that of the staged group.Compared with beforeoperation,Qmax increased significantly,and RU,IPSS scores,maximum urethral closure pressure and maximum urethral pressure decreased significantly in both groups 6 months after operation.Qmax of the concurrent group was higher than that of the staged group,and RU,IPSS scores,maximum urethral closure pressure and maximum urethral pressure were lower,The difference is statistically significant(P<0.05).The recurrence rate of bladder cancer and the implantation and metastasis rate of prostate fossa were lower in the con current group than in the staged group(P<0.05).Conclusions For patients with bladder cancer complicated with prostatic hyperplasia,simultaneous TURBT and PKRP are effective,and the risk of tumor recurrence and metastasis is relatively low.
