The prevention and cure of chylous after neck dissection in thyroid carcinoma

Abstract:Objective To explore the clinical analysis and management of chylous after neck dissection in thyroid carcinoma. Methods Retrospective analysis of 35 cases chylous after neck disseetion in thyroid carcino-ma from January 2010 to December 2014 in our hospitul were analyzed retrospectively. Results The incidence of chylous leakage after dissection neck lymph nodes during thyroid cancer surgery is about 3. 22%, including 27 cases on the left lateral(77. 14%), 15 cases recovered under conservative treatment, 20 cases recovered by operation. Conclusion The high incidence of chylous leakage on the left lateral is related to anatomical fac-tors. Spot chylous fistulae in operation is the key to prevent the complication. Operation is appropriate in case of massive leakage( >500 ml per day) or failure in 1-week conservative treatment. Success rates could be in-creased by placing peripheral muscle on fistulae.
