High-flow nasal cannula oxygen therapy for acute respiratory failure in chronic obstructive pulmonary disease

Abstract:Over the past decade,high-flow nasal cannula(HFNC)oxygen therapy has been widely applied in treatment of respiratory failure.Compared with conventional oxygen therapy(COT),HFNC oxygen therapy can maintain stable fraction of inspired oxygen(FiO2),not only improve arterial oxygen partial pressure but also reduce arterial carbon dioxide partial pressure,decrease work of breathing,and improve diaphragmatic function.Clinical researches have confirmed that HFNC oxygen therapy is superior to COT in treating acute respiratory failure in chronic obstructive pulmonary disease(COPD).However,it remains controversial whether HFNC oxygen therapy is non-inferior to non-invasive ventilation(NIV).More large-scale,prospective,randomized controlled clinical trials are needed to optimize the treatment protocols and provide high-grade evidence-based medical support for HFNC oxygen therapy in treating acute respiratory failure in COPD.
