Abstract：Elderly patients with multiple chronic conditions are highly susceptible to medication discrepancies during the hospital-home transition,which can lead to a series of adverse drug events,including an increased rate of rehospitalization.This study systematically reviewed issues related to medication discrepancies in the elderly patients,focusing on their causes,assessment tools and influencing factors,so as to provide theoretical support and practical guidance for improving medication safety management during the special period.
