Construction and application of an operational pain management protocol for endotracheal suctioning in mechanically ventilated adult patients

Abstract:Objective To develop an operational pain management protocol for suctioning in mechanically ventilated adults and evaluate its clinical efficacy.Methods Mechanically ventilated patients admitted to the intensive care unit(ICU)of the Third People's Hospital of Hubei Province were selected as research subjects.A total of 208 patients treated in the first half of 2024 were assigned to the control group and received conventional suctioning procedures,while another 208 patients admitted in the second half of 2024 were chosen into the observation group and given an operational pain management protocol constructed based on evidence-based summary and Delphi method.The behavioral pain scale(BPS)scores were compared between the two groups at 1 min before suctioning,during suctioning,and at 1 min,5 min,and 15 min after suctioning.The incidence of airway mucosal injury,induced cough,and suctioning intervals were also compared.Results The BPS score of the observation group was significantly lower than that of the control group(Fintergroup=710.209,P<0.001).Moreover,the incidence of airway mucosal injury and irritating cough of the observation group were significantly lower than the latter group(χ2=17.903,P<0.001;χ2=20.779,P<0.001).Meanwhile,the suctioning interval in the observation group was significantly longer than the latter(t=-9.982,P<0.001).Conclusion The protocol in this study is safe and feasible.It can reduce the degree of suctioning pain and provide a reference for clinical management of suctioning pain in patients undergoing mechanical ventilation.
