Application of hospital-community-family three-element integrated management in patients with chronic heart failure

Abstract：Objective To explore the effect of hospital-community-family integrated management on patients with chronic heart failure(CHF).Methods A total of 118 CHF patients admitted to the department of cardiology of geriatric medicine between March 2022 and March 2024 were selected,and randomly divided into a control group and an intervention group,each of 59,according to a random number table.The control group was given routine nursing,while the intervention group was provided with hospital-community-family integrated management.Then,the health behavior,heart function and life quality were compared between the two groups.Results After the intervention,the health behavior in the intervention group were significantly higher than that of the control group(t=4.111,P＜0.001),and the distance of 6 min walking test was significantly longer than the latter group(t=3.057,P=0.003).Moreover,the level of aminoterminal brain natriuretic peptide precursor and the scores of Minnesota living with heart failure questionnaire were significantly lower than those of the control group(t=18.276,P＜0.001;t=9.590,P＜0.001),while the left ventricular ejection fraction was higher than the latter(t=2.669,P=0.009).Conclusion For CHF patients,the hospital-community-family integrated management can improve their self-care ability,enhance their health behavior,and improve their heart function and life quality.
