Abstract:Objective To investigate the effect of continuity care on self-management and health status of patients with chronic kidney diseases (CKD).Methods Totally 180 CKD patients admitted to our hospital between April 2012 and March 2014 were selected and randomly divided into an observation group (n=90)and a control group (n=90),according to a random number table. Both groups received routine care during hospitalization,while the observation group was additionally given continuity care after discharge.The self management and health status of both groups were compared before discharge and six months later.Results There was no statistical difference between 2 groups in self-management and health status at discharge. And six mouths later, the self-management and health status in the observation group were better than those in the control group.Conclusion Continuity care is of great significance in promoting self-management skills and health status of CKD patients.
