Abstract：Objective To investigate the clinical efficacy of the continuity of care measures on the rehabilitation of chronic heart failure (CHF)patients.Methods A total of 180 CHF patients admitted between April 2012 and March 201 5 were divided into an observation group (n =90)and a control group (n = 90 ), according to a random number table. Both groups were given the conventional therapy and nursing during hospitalization, while the observation group was additionally provided with continuity of care after discharge.One year later,cardiac functional grading,color Doppler ultrasound of heart,six-minute walking test (SMWT)and life quality were conducted for both groups.Results After a year of continuity of care,significant improvement was observed in the heart function,stroke volume of echocardiography,left ventricular ejection fraction and life quality of the observation group than the control group.Conclusions Continuity of care can help to improve heart function,heart pumping and life quality of CHF patients.
