Observation on the effect of post-discharge continuing nursing for the elderly patients with chronic diseases

Abstract:Objective To investigate the effect of continuing nursing intervention on prognosis of elderly patients with chronic diseases after their discharges.Method Seventy-two elderly discharged patients with chronic diseases,hospitalized in our hospital from June to December 2014 were chosen and divided randomly into a control group and an intervention group,with each group of 36 patients.The control group was given the routine nursing guidance during 3 months after their discharges, while the intervention group was provided with continuing nursing guidance, namely telephone interviews and family nursing guidance in addition to the routine nursing guidance. The illness condition,activities of daily living (ADL),re-hospitalization and survival rate for both groups were compared before and after 3 months of their discharge.Results The illness condition and ADL of the intervention group was significantly better than those of the control group (Z =-3.675,P <0.01),while the re-hospitalization rate of the intervention group was significantly lower than that of the control group(χ2 =-1 1.70,P <0.01).Conclusions The continuing nursing can alleviate the illness of the elderly patients with chronic diseases, reduce their times of re-hospitalization,and improve their ability of life,which in turn enhances their quality of life.
