Abstract:Objective To analyze the efficacy stratification and corresponding influencing factors of the first 131 I treatment after surgery for intermediate-to high-risk differentiated thyroid cancer. Methods A total of 90 patients with intermediate-to high-risk differentiated thyroid cancer who underwent surgery in our hospital from June 2020 to June 2023 were selected as the study subjects.According to the initial dose of 131 I,they were divided into three groups:low dose group(1.11~3.70 GBq),medium dose group(3.70~5.55 GBq),and high dose group(5.55~7.40 GBq).The clinical efficacy and its related influencing factors of 131 I treatment were analyzed. Results After 131 I treatment,90 patients underwent follow-up examination at(4±1)months,and the results showed that 62 cases achieved successful thyroid remnant ablation, with a success rate of 68.89%.There were statistically significant differences(P<0.001)in the thyroid surgery method, time to surgery, and serum TSH level before treatment between patients with successful and unsuccessful thyroid remnant ablation. The thyroid surgery method(OR=7.584,P<0.001),the time to surgery(OR=0.721,P<0.001),and the serum TSH level before treatment(OR=8.428,P<0.001)were independent influencing factors for the efficacy of the first 131 I treatment after thyroid cancer surgery. Conclusion The type of thyroid surgery, surgical time, and serum TSH level before treatment are important factors affecting the efficacy of the first 131 I treatment for thyroid cancer after surgery.
