Comparative study on the therapeutic effects of different middle turbinate treatments in endoscopic sinus surgery for chronic sinusitis with nasal polyps

Abstract:Objective To investigate the efficacy of different middle turbinate treatments in endoscopic sinus surgery for chronic sinusitis with nasal polyps(CRSwNP).Methods 118 patients with CRSwNP admitted to Jiaozhou Hospital,East Hospital Affiliated to Tongji University from June 2020 to September 2022 were divided into control group and observation group according to the treatment method of middle turbinate,with 59 cases in each group.Both groups underwent endoscopic sinus surgery,in which the control group retained the middle turbinate,while the observation group partially removed the middle turbinate.The two groups were compared in terms of treatment efficacy,nasal function[nasal volume(NCV),minimum nasal cross-sectional area(NMCA),distance from minimum nasal cross-sectional area to anterior nostril(DCAN)and nasal airway resistance(NAR)],Lund-Kennedy nasal endoscopy score,SNOT-20 score,and incidence of intraoperative cavity adhesion.Results The total effective rate of the observation group was higher than that of the control group(98.31%vs 86.44%,χ2=4.330,P=0.037).Three months after surgery,the DCAN,NAR,Lund-Kennedy nasal endoscopy scores,and SNOT-20 scores in the observation group were lower than those in the control group[(1.58±0.13)vs(1.69±0.17),(0.25±0.04)vs(0.36±0.09),(2.17±0.41)vs(3.42±0.66),(10.15±1.13)vs(13.84±1.71),respectively,P<0.05],while NCV and NMCV were higher than those in the control group[(19.48±2.26)vs(15.46±1.93),(0.59±0.10)vs(0.52±0.08),respectively,P<0.05].After 1 year of postoperative follow-up,the incidence of intraoperative cavity adhesion in the observation group was lower than that in the control group(5.08%vs 20.34%,χ2=6.186,P=0.013).Conclusion Compared with preserving the middle turbinate,partial removal of the middle turbinate during endoscopic sinus surgery for CRSwNP can improve clinical efficacy,promote nasal mucosal repair,reduce postoperative cavity adhesion,and improve the nasal function and quality of life in patients.
