Predictive value of serum thyroglobulin for recurrence/metastasis following radical surgery in differentiated thyroid carcinoma

Abstract：Objective To evaluate the predictive value of serum thyroglobulin(Tg)levels in identifying disease recurrence or metastasis in patients with differentiated thyroid cancer(DTC)after radical surgery.Methods 358 patients who underwent DTC radical surgery were divided into the recurrence group and the non-recurrence group based on whether there was recurrence within one year after surgery.The clinical data were compared between the two groups.Logistic regression analysis was used to identify the relevant factors associated with the recurrence or metastasis in DTC radical surgery patients.Receiver operating characteristic(ROC)curves were plotted to analyze the predictive value of serum Tg,TgAb and their combination for recurrence or metastasis in DTC radical surgery patients.Results Compared with the non-recurrence group,patients in the recurrence group had higher proportion of TNM stages Ⅲ-Ⅳ,capsular invasion and higher levels of Tg,TSH and TgAb(all P<0.05).Logistic regression analysis revealed that TNM stages Ⅲ-Ⅳ,high levels of Tg and TgAb were risk factors for recurrence or metastasis in patients undergoing DTC radical surgery(P<0.05).ROC curve analysis showed that the areas under the curve(AUC)for serum Tg,TgAb and their combination in predicting recurrence or metastasis were 0.877,0.805 and 0.913,respectively.The corresponding sensitivities were 67.25％,60.25％and 75.00％,and the specificities were 90.50％,80.00％and 90.50％.Conclusions Serum Tg and TgAb are independent risk factors for recurrence or metastasis in patients undergoing DTC radical surgery.Moreover,the combination of Tg and TgAb significantly improves the predictive performance.
