Observation of the therapeutic effect of intervertebral foramen endoscopic technique in treating lumbar spinal stenosis

Abstract:Objective To observe and analyze the clinical efficacy of minimally invasive decompression technology assisted by intervertebral foramen endoscopy in the treatment of degenerative lumbar spinal stenosis.Methods 24 months of prospective data were collected from 45 consecutive patients with degenerative lumbar spinal stenosis who underwent FE surgery(interlaminar approach)in our hospital from January 2023 and December 2024.The mean age of the 24 female and 21 male patients was 72.35±9.24 years.The surgical outcomes were assessed using the visual analog scale,Oswestry Disability Index and modified MacNab criteria.We used the interlaminar Percutaneous Endoscopic Decompression technique to treat the patients.The decompression could be performed using a burr and punches under endoscopic control and fluoroscopic control.At the last time of postoperative follow-up,the patients were all studied with flexion-extension lateral films to determine the stability of the lumbar spine.Results There were no injuries to nerves,or dura mater,and no cauda equina syndrome occurred.One patient developed a transient postoperative dysesthesia,and one patient experienced transient urinary retention.The mean visual analog scale improved from 6.96±0.95 at baseline to 3.29±0.55 at 3 months,2.67±0.48 at 6 months,2.27±0.45 at 12 months,and 2.09±0.29 at 24 months post-surgery(P<0.05).The mean Oswestry Disability Index improved from 87.60±6.15 at baseline to 38.76±6.52 at 3 months,29.89±8.07 at 6 months,27.33±7.99 at 12 months,26.60±7.78 at 24months post-surgery(P<0.05).Based on the modified MacNab criteria,excellent or good results were obtained in 88.89%of the patients.No instability was found in the patients.Conclusion The minimally invasive decompression technique assisted by intervertebral foramen endoscope is a safe and effective treatment for degenerative lumbar spinal stenosis,with satisfactory results.Especially for elderly patients,this method fully utilizes the advantages of modern advanced minimally invasive techniques in the treatment of lumbar degenerative diseases.
