Subjective and objective therapeutic effect of nasal cavity expansion surgery for the treatment of nasal obstruction in the self-rating anxiety scale

Abstract:Objective To explore subjective and objective efficacy evaluation criteria of nasal cavity expansion surgery for the treatment of nasal obstruction complicated with structural abnormality of nasal cavity,and to provide the basis for the surgical evaluation system.Methods The polysomnography(PSG),nasal airway resistance,SNOT-20 life quality scale(SNOT-20)and self-rating anxiety and depression scale in 32 patients with nasal obstruction were examined before operation.All patients were treated with nasal endoscopic expansion,and were followed up for 3 to 6 months.The index changes between before and after operation among groups were compared.Results The scores of SNOT-20 life quality,anxiety and depression scales significantly decreased after operation[(52.28±3.38 vs 26.47±2.05,28.72±1.33 vs 22.75±0.45,29.56±1.39 vs 23.28±0.56,(P<0.00)).The nasal resistance was significantly decreased after the operation[(6.31±0.190)kPa·s/L vs(1.24±0.21)kPa·s/L,P<0.01].Compared with indexes before operation,only the LSaO2 was increase[(81.41±1.94 vs 86.41±1.15),P<0.002)],the apnea hypopnea index(AHI)and sleep efficiency did not change significantly(P>0.05).For the relationship between anxiety scale and AHI through box plots,the high anxiety score was in the severe OSAHS group(AHI≥30)before operation.During post-operation follow-up,the greater the AHI was,the more significantly the median of anxiety scores decreased,in comparison with that before operation,but high scores still existed.Conclusions Cavity expansion surgery can improve nasal cavity ventilator function caused by nasal obstruction,proved by the evidence of decreased subjective scale scores and objective nasal resistance.The score of self-rating anxiety scale of nasal obstruction complicated with severe OSAHS may be high before operation.It can be improved significantly,but the high anxiety score may still remain after operation.Psychological evaluation can be used as a subjective observation index of surgical efficacy.
