A case report and analysis of rhabdomyolysis Induced by voriconazole combined with omeprazole

Abstract:A 71-year-old female patient was admitted to on January 18,2024,due to"intermittent upper abdominal discomfort for more than 10 years and jaundice for 1 month".After admission,relevant examinations were completed,and she was diagnosed with liver damage and acute-on-chronic liver failure.On January 22,2024,due to pulmonary infection with fever,sputum culture results indicated Aspergillus fumigatus infection,and voriconazole was administered for treatment.On January 28,2024,omeprazole was administered for gastric discomfort.Shortly after the combined treatment of voriconazole and ome-prazole,the patient developed symptoms of neuromuscular injury,with a sharp rise in creatine kinase and myoglobin levels.No other disease factors that might cause such symptoms were found.Based on the aforementioned clinical manifestations and a de-tailed analysis of the patient's medication history,it was strongly suspected that the muscle injury was induced by voriconazole.Considering the disease course,medication status and the consultation opinions from the pharmacy department,rhabdomyolysis was suspected,and a potential association with voriconazole could not be ruled out.After drug discontinuation,the patient's symptoms alleviated,and the levels of creatine kinase and myoglobin returned to normal.Therefore,attention should be paid to drug-drug interaction risks when voriconazole is co-administered with other medications,particularly for serious and rare ad-verse reactions such as rhabdomyolysis.
