Effects of dapagliflozin on hemodynamics and renal function indexes in patients with T2DN

Abstract:Objective To investigate the effects of dapagliflozin on renal hemodynamics and renal function parame-ters in patients with type 2 diabetic nephropathy(T2DN).Methods A total of 92 patients with T2DN admitted to Zhoushan Pu-tuo District People's Hospital from January 2023 to December 2024 were retrospectively analyzed and divided into the exposure group(n=49)and the non-exposure group(n=43).The non-exposure group received oral metformin hydrochloride tablets,while the exposure group received oral dapagliflozin tablets.Both groups were treated for 12 weeks.The efficacy,glycemic parameters,renal function indexes and renal hemodynamic assessment parameter(vascular resistance index(RI))were compared between the exposure group and the non-exposure group.Results The total effective rate in the exposure group was 87.76%(43/49),higher than 60.47%(26/43)in the non-exposure group(χ2=9.097,P=0.003).After treatment,fasting blood glucose and glycosylated hemoglobin in the exposure group were lower than those in the non-exposure group((6.83±0.92)mmol/L vs(8.15±1.24)mmol/L,t=5.843,P<0.001;(6.70±0.75)%vs(7.89±0.98)%,t=6.584,P<0.001).Glomerular filtration rate was improved in the exposure group compared with that in the non-exposure group((108.42±15.63)mL/min vs(119.25±12.71)mL/min,t=3.614,P<0.001).After treatment,blood urea nitrogen,serum creatinine,urinary protein-to-creatinine ratio,and urinary pro-tein were reduced in the exposure group compared to the non-exposure group((4.97±0.82)mmol/L vs(5.36±0.88)mmol/L,t=2.200,P=0.030;(75.55±8.43)μmol/L vs(81.26±8.57)μmol/L,t=3.216,P=0.002;(0.18±0.03)g/24 h vs(0.22±0.05)g/24 h,t=4.717,P<0.001;(21.09±3.61)g/mol vs(25.17±3.35)g/mol,t=5.593,P<0.001).In terms of renal hemodynamics,the exposure group showed a lower post-treatment RI compared to the non-exposure group((0.58±0.07)vs(0.62±0.09),t=2.394,P=0.019).Conclusion Dapagliflozin can significantly improve the renal RI in T2DN patients,reduce hyperfiltration,decrease urinary protein excretion,and protect renal function,demonstrating superior efficacy compared to metformin.
