Clinical evaluation of levofloxacin combined with the 3HRZE regimen in the treatment of chronic tuberculous empyema

Abstract:Objective To analyze the efficacy and safety of levofloxacin combined with the 3HRZE regimen in sur-gical patients with chronic tuberculous empyema.Methods A retrospective cohort study was conducted.A total of 82 patients undergoing surgery for chronic tuberculous empyema admitted to the Fifth People's Hospital of Suzhou between January 2022 and May 2025 were included in this analysis,with their clinical data extracted from the hospital's medical record system.Based on the actual treatment regimens documented in the medical records,patients were categorized into the 3HRZE group and the combination group.The 3HRZE group received basic treatment plus the 3HRZE regimen(41 cases),while the combination group received ba-sic treatment plus levofloxacin combined with the 3HRZE regimen(41 cases).Both groups underwent continuous treatment for 3 months.Efficacy,immune function,length of hospital stay,and the incidence of adverse reactions were extracted from the medical records for the 3HRZE group and the combination group for comparative analysis.Results The total effective rate was higher in the combination group than that in the 3HRZE group(92.68%(38/41)vs 75.61%(31/41)),with a statistically significant difference(χ2=4.479,P<0.05).The combination group showed higher CD4+percentage and levels of immunoglobulin A,and lower CD8+percentage,compared to the 3HRZE group((37.42±4.25)%vs(34.31±4.47)%,(2.54±0.42)g/L vs(2.27±0.39)g/L,(27.10±3.78)%vs(29.97±3.54)%),with statistically significant differences(t=3.229,3.016,-3.548,all P<0.05).The length of hospital stay was short-er in the combination group than that in the 3HRZE group((12.01±4.01)days vs(15.23±3.36)days),with a statistically significant difference(t=-3.941,P<0.05).The incidence of adverse reactions in the combination group and the 3HRZE group was 14.63%(6/41)and 12.20%(5/41),respectively,with no statistically significant difference(χ2=0.105,P>0.05).Conclusion For patients un-dergoing surgery for chronic tuberculous empyema,levofloxacin combined with the 3HRZE regimen demonstrated better treatment efficacy,greater improvement in immune function,and a shorter hospital stay compared to the control group.
