Influencing factors on the efficacy of secukinumab in the treatment of moderate to severe plaque psoriasis

Abstract:Objective To investigate the factors influencing the efficacy of secukinumab in the treatment of patients with moderate to severe plaque psoriasis.Methods A retrospective controlled study design was adopted.A total of 102 pa-tients with moderate-to-severe plaque psoriasis treated with secukinumab at Shanghai Skin Disease Hospital from June 2022 to August 2024 were selected.Patients were grouped based on their Psoriasis Area and Severity Index(PASI)improvement rate after 2 months of treatment.Patients with a PASI improvement rate of 50%to<75%were classified into the PASI 50 group,those with 75%to<90%into the PASI 75 group,and those with 90%to 100%into the PASI 90 group.The time to achieve the respective PASI improvement threshold and clinical data were recorded,and multivariate ordinal logistic regression analysis was employed to identify factors influencing treatment efficacy.Results Patients were categorized as follows:74 into the PASI 50 group,18 into the PASI 75 group,and 10 into the PASI 90 group.The treatment course for the 102 patients ranged from 24 to 97 weeks.The time to achieve PASI 50,PASI 75,and PASI 90 improvement rates for all patients was(4.89±1.33),(12.37±3.62),and(35.37±8.36)weeks,respectively.No statistically significant difference was found in the PASI improvement rate after 2 months of secukinumab treatment among patients with moderate to severe plaque psoriasis in terms of age,gender,smoking history,family history of psoriasis,dermatology life quality index,and seasonal exacerbation patterns(F/χ²=0.215,0.330,0.731,1.631,1.242,0.497,all P>0.05).Statistically significant differences in PASI improvement rate were found across different body mass index(BMI)categories,disease durations,comorbidities,histories of prior biologic therapy,medication adherence levels,and baseline PASI scores(F/χ²=3.827,6.637,7.258,6.720,7.789,6.458,all P<0.05).Multivariate ordinal logistic regression analysis revealed that BMI(OR=0.760,95%CI:0.614-0.943),disease duration(OR=0.258,95%CI:0.133-0.503),comorbidities(OR=10.730,95%CI:1.657-69.477),history of prior biologic therapy(OR=7.622,95%CI:1.237-46.993),medication adher-ence(OR=5.865,95%CI:1.435-23.999),and baseline PASI score(OR=0.712,95%CI:0.578-0.876)all significantly influenced the efficacy of secukinumab in patients with moderate to severe plaque psoriasis(all P<0.05).Conclusion Secukinumab demonstrates rapid onset and sustained efficacy in the treatment of moderate to severe plaque psoriasis.BMI,disease dura-tion,comorbidities,history of prior biologic therapy,medication adherence,and baseline PASI score all significantly influence treatment outcomes.
