Clinical evaluation of octreotide combined with esomeprazole in treating peptic ulcer bleeding

Abstract:Objective To analyze the efficacy of octreotide combined with esomeprazole in the treatment of pep-tic ulcer bleeding(PUB),and to compare the changes in the levels of serum neutrophil-to-lymphocyte ratio(NLR)and plate-let-to-lymphocyte ratio(PLR).Methods A prospective controlled study design was used.A total of 105 patients with PUB ad-mitted to Huainan Oriental Hospital Group General Hospital from January 2022 to January 2025 were included and divided into the control group(n=52,treated with esomeprazole)and the combined group(n=53,treated with esomeprazole plus octreotide)according to simple random sampling.The clinical efficacy between the two groups was compared,as well as the levels of serum NLR,PLR,and interleukins(IL)2,IL-6,IL-17 before and after treatment,the four coagulation indicators,and adverse reactions.Results The total effective rate of the combined group was significantly higher than that of the control group(96.23%(51/52)vs 82.69%(43/52))(χ2=5.126,P<0.05).The hospitalization time and hemostasis time of the combined group were both shorter than those of the control group((5.40±1.17)d,(23.46±3.33)h vs(7.59±2.28)d,(32.13±5.08)h)(t=6.209,10.362,both P<0.05),and the blood transfusion volume of the combined group was lower than that of the control group((284±56)mL vs(418±62)mL)(t=11.627,P<0.05).The prothrombin time,activated partial thromboplastin time and thrombin time in the combined group were shorter than those in the control group((9.40±1.17)s,(22.59±2.73)s,(12.76±1.65)s vs(11.61±1.68)s,(27.53±2.86)s,(15.73±2.78)s)(t=7.834,9.055,6.672,all P<0.05),and the level of fibrinogen in the combined group was higher than that in the control group((2.81±0.38)g/L vs(2.12±0.27)g/L)(t=10.742,P<0.05).The IL-2 level in the combined group was higher than that in the control group((3.70±0.71)ng/mL vs(2.81±0.66)ng/mL)(t=6.650,P<0.05),while the levels of PLR,NLR,IL-6 and IL-17 in the combined group were lower than those in the control group((5.40±0.72),(22.63±3.68),(33.29±3.63),(169.36±17.65)pg/mL vs(6.37±0.84),(26.37±4.28),(37.83±3.86),(182.27±20.78)pg/mL)(t=4.804,6.357,6.210,3.433,all P<0.05).The incidence of adverse reactions was 1.89%(1/53)in the combination group and 9.62%(5/52)in the control group,and the difference was not statistically significant(χ2=2.748,P=0.097).Conclusion Octreotide combined with esomeprazole has a significant effect in the treatment of PUB,and can improve coagulation function,regulate NLR and PLR levels,reduce inflamma-tory responses,without increasing the risk of adverse events.
