Efficacy of calcium dobesilate combined with enalapril in patients with non-dipper hypertension complicated with type 2 diabetes mellitus

Abstract:Objective To investigate the clinical efficacy of calcium dobesilate combined with enalapril in the treat-ment of patients with non-dipper hypertension complicated with type 2 diabetes mellitus(T2DM).Methods A prospective controlled study design was used.A total of 80 patients with non-dipper hypertension complicated by T2DM treated at Shou-guang People's Hospital from December 2022 to December 2024 were enrolled and randomly divided into two groups using a random number table.The control group(40 cases)received foundational medication plus enalapril,while the observation group(40 cases)received foundational medication plus enalapril plus calcium dobesilate.Both groups continued treatment for 6 months.Clinical efficacy,blood pressure rhythm changes,blood glucose levels,pancreatic islet function,vascular endothelial function,and adverse reactions were compared between the two groups.Results After treatment,the total effective rate in the obser-vation group was 92.50%(37/40),higher than that in the control group(75.00%(30/40))(χ2=4.501,P<0.05).After treatment,the daytime systolic and diastolic blood pressures and nighttime systolic and diastolic blood pressures in the observation group((126.03±6.31),(81.31±6.29),(115.62±6.40),(75.65±5.65)mmHg(1 mmHg=0.133 kPa),respectively)were lower than those in the control group((141.36±6.94),(87.54±6.27),(124.33±6.38),(80.31±5.40)mmHg,respectively)(t=10.337,4.437,6.096,3.771,all P<0.05).The nocturnal blood pressure decline rate in the observation group((12.02±2.18)%)was higher than that in the control group((7.53±1.79)%)(t=10.067,P<0.05).After treatment,the levels of fasting blood glucose,2-hour postprandial blood glucose,and glycated hemoglobin in the observation group((6.08±0.57)mmol/L,(9.57±0.56)mmol/L,(6.12±0.51)%,respectively)were lower than those in the control group((6.95±0.67)mmol/L,(10.28±0.91)mmol/L,(7.43±0.96)%,respective-ly)(t=6.255,4.203,7.622,all P<0.05).After treatment,the levels of fasting insulin and homeostasis model assessment of insulin resistance index in the observation group((11.22±1.63)mU/L,(3.36±0.35),respectively)were lower than those in the control group((13.01±1.76)mU/L,(3.91±0.38),respectively)(t=4.719,6.733,both P<0.05).After treatment,the endothelin-1 level in the ob-servation group((44.74±6.82)pg/mL)was lower than that in the control group((55.89±6.75)pg/mL)(t=7.349,P<0.05),while the nitric oxide level in the observation group((86.11±11.52)μmol/L)was higher than that in the control group((71.25±10.86)μmol/L)(t=5.936,P<0.05).The incidence of adverse reactions was 15.00%(6/40)in the observation group and 12.50%(5/40)in the con-trol group,with no statistically significant difference(χ²=0.105,P>0.05).Conclusion The combination of calcium dobesilate and enalapril demonstrates good efficacy in treating non-dipper hypertension associated with T2DM.It can effectively improve the patients'circadian blood pressure rhythm,regulate blood sugar levels,ameliorate insulin resistance,promote the repair of vascular endothelial function,and exhibit a favorable safety profile.
