Efficacy of evolocumab combined with atorvastatin in patients with acute myocardial infarction

Abstract:Objective To explore the clinical efficacy of the combination of evolocumab and atorvastatin in the treatment of acute myocardial infarction(AMI).Methods A retrospective controlled study method was adopted.A total of 120 AMI patients admitted to Puyang Oilfield General Hospital from January 2022 to October 2024 was selected and divided into two groups based on different treatment regimens.The statin group received atorvastatin treatment,and the combination group received evolocumab in addition to atorvastatin treatment.The clinical efficacy,lipid metabolism indicators,cardiac func-tion indicators,inflammatory marker levels before and after treatment,and the occurrence of adverse reactions were collected,extracted,and compared between the two groups.Results The total effective rate in the combination group was higher than that in the statin group(93.22%(55/59)vs 70.49%(43/61),χ2=10.348,P=0.001).After treatment,total cholesterol,triglycerides,and low-density lipoprotein cholesterol were all reduced in both groups,and the levels in the combination group((3.02±0.42),(1.16±0.21),(1.65±0.24)mmol/L)were lower than those in the statin group((3.92±0.59),(1.92±0.30),(2.62±0.38)mmol/L)(t=9.598,16.027,16.655,all P<0.05).After treatment,left ventricular ejection fraction(LVEF),left ventricular end-systolic di-ameter(LVESD),and left ventricular end-diastolic diameter(LVEDD)improved in both groups.LVEF in the combination group was(66.15±5.29)%,higher than that in the control group((56.48±4.51)%),while LVESD and LVEDD in the combination group((37.03±3.33),(46.20±4.16)mm)were lower than those in the statin group((40.95±3.69),(49.12±3.92)mm),with statistically significant differences(t=10.788,6.103,3.958,all P<0.05).After treatment,serum tumor necrosis factor-α and high-sensitivity C-reactive protein decreased in both groups,and the levels in the combination group((1.25±0.31)ng/mL,(4.60±1.15)mg/L)were lower than those in the statin group((1.64±0.42)ng/mL,(8.82±2.21)mg/L),with statistically significant differences(t=5.772,13.056,both P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(13.11%(8/61)vs 25.42%(15/59),χ2=2.933,P>0.05).Conclusion Evolocumab combined with atorvastatin can improve the clinical efficacy in AMI patients,reduce lipid levels,improve cardiac function and inflammatory levels,with a good safety profile.
