Clinical evaluation of trimetazidine combined with metoprolol in the treatment of ischemic cardiomyopathy complicated with hypertension

Abstract：Objective To evaluate of the effect of trimetazidine adjunctive to metoprolol on cardiac function and in-flammatory levels in patients with ischemic cardiomyopathy complicated by hypertension.Methods A prospective controlled study was conducted.A total of 82 patients with ischemic cardiomyopathy and hypertension from Huai'an First Hospital Affil-iated to Nanjing Medical University between January 2023 and June 2024 were enrolled and randomly divided into the control group(41 cases,receiving metoprolol monotherapy)and the observation group(41 cases,receiving trimetazidine combined with metoprolol)using a random number table.After 12 weeks of continuous treatment,cardiac function indices,serum inflammatory markers,and incidence of adverse reactions were compared between the two groups.Results After treatment,the observa-tion group showed lower left ventricular end-diastolic diameter and left ventricular end-systolic diameter((51.12±4.28)mm,(39.51±3.22)mm,respectively)compared to the control group((56.97±5.41)mm,(43.31±4.32)mm,respectively),while the left ventricular ejection fraction was higher in the observation group than in the control group((45.69±3.41)%vs(40.35±3.32)%)(t=5.430,4.516,7.184,all P<0.05).After treatment,the observation group showed lower levels of high-sensitivity C-reactive protein,tumor necrosis factor-α,and interleukin-6((45.59±12.14)mg/L,(50.92±12.78)ng/L,(14.54±3.15)ng/mL,respectively)compared to the control group((52.27±12.51)mg/L,(57.16±12.31)ng/L,(16.25±3.21)ng/mL,respectively)(t=2.454,2.252,2.435,all P<0.05).The incidence of adverse reactions in the observation group was 14.63%(6/41),showing no statistically significant difference compared to 12.20%(5/41)in the control group(χ2=0.105,P>0.05).Conclusion The combination of trimetazidine and metoprolol demonstrates better improvements in cardiac function and inflammatory markers in patients with ischaemic cardiomyopathy complicated by hypertension,with a favorable safety profile.
