Additive effect of ezetimibe in nicorandil-treated patients with unstable angina pectoris and comorbid dyslipidemia

Abstract:Objective To investigate the additive effect of ezetimibe on lipid profile and cardiac function in patients with unstable angina pectoris(UAP)and dyslipidemia treated with standard nicorandil-based therapy.Methods A total of 108 patients with UAP and dyslipidemia admitted to the Department of Cardiology,Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine in Hebei Province between April 2023 and February 2024 were randomly divided into the ob-servation group(54 cases)and the control group(54 cases)using a random number table method.All patients received conven-tional therapy.The control group received additional nicorandil,while the observation group received nicorandil combined with ezetimibe.Angina symptoms,lipid profiles,vascular endothelial function,left ventricular function,and adverse drug reactions were compared between the two groups.Results After treatment,the observation group had a lower frequency((1.48±0.50)times/week vs(2.35±0.48)times/week)and shorter duration((3.26±0.44)min/episode vs(5.39±0.60)min/episode)of angina attacks than the control group(t=9.168,21.082,both P<0.05).The total effective rate for electrocardiogram improvement was higher in the ob-servation group(88.89%(48/54)vs 72.22%(39/54),χ2=4.788,P<0.05).After treatment,the observation group showed lower left ventricular end-diastolic diameter and left ventricular end-systolic diameter((52.71±4.63),(36.52±4.54)mm)than the control group((55.46±4.18),(40.11±4.21)mm),while the left ventricular ejection fraction level in the observation group((54.63±6.62)%)was higher than that in the control group((51.24±6.15)%)(t=3.239,4.261,2.761,all P<0.05).After treatment,the observa-tion group also showed more significant improvements in levels of triglyceride,total cholesterol,and low-density lipoprotein cholesterol((1.14±0.35),(4.51±0.72),(1.74±0.59)mmol/L)compared to the control group((1.95±0.36),(4.92±0.74),(3.26±0.53)mmol/L),while the high-density lipoprotein cholesterol level in the observation group((3.11±0.68)mmol/L)was higher than that in the control group((2.25±0.56)mmol/L)(t=11.857,2.922,14.105,7.162,all P<0.05).After treatment,the observa-tion group showed higher nitric oxide levels((50.20±6.18)μmol/L)compared to the control group((42.91±5.91)μmol/L),and lower endothelin-1 levels((4.37±1.17)pg/mL)than the control group((4.84±1.23)pg/mL)(t=6.265,2.035,both P<0.05).The incidence of adverse reactions did not differ significantly between the observation group and the control group(12.96%(7/54)vs 11.11%(6/54))(χ2=0.087,P>0.05).Conclusion The combination of ezetimibe and nicorandil showed an additive effect in en-hancing the improvement of blood lipid profiles,vascular endothelial function,and left ventricular function in patients with UAP and dyslipidemia.
