Evaluation of recombinant human brain natriuretic peptide combined with torasemide in treating coronary heart disease complicated by acute left heart failure

Abstract:Objective To investigate the efficacy of recombinant human brain natriuretic peptide(rhBNP)and torasemide in patients with coronary heart disease(CHD)complicated by acute left heart failure(ALHF)and their impact on inflammatory markers.Methods A prospective randomized controlled design was adopted.A total of 148 patients with CHD complicated by ALHF treated at Linfen Central Hospital from June 2024 to June 2025 were enrolled and randomly divided into the control group and the experimental group using a random number table method,with 74 cases in each group.The control group received conventional treatment plus torasemide,while the experimental group received conventional treatment combined with torasemide and rhBNP.Both groups were treated continuously for 7 days.Clinical efficacy,cardiac function parameters,inflammatory markers,relevant laboratory indices,blood pressure,heart rate,and adverse reactions were compared between the two groups.Results The total effective rate in the experimental group was higher than that in the control group(91.89%(68/74)vs 79.73%(59/74),χ2=4.495,P<0.05).After treatment,the levels of systolic blood pressure,diastolic blood pressure,and heart rate in the experimental group((108.26±10.15)mmHg(1 mmHg=0.133 kPa),(76.33±7.42)mmHg,(70.07±7.12)beats/min,respectively)were all lower than those in the control group((114.02±10.73)mmHg,(81.52±8.01)mmHg,(75.13±7.56)beats/min,respectively)(t=3.355,4.089,4.191,all P<0.05).After treatment,the levels of left ventricular end-diastolic diameter and left ventricular end-systolic diameter in the experimental group((52.38±4.78)mm and(48.37±4.51)mm,respectively)were lower than those in the control group((56.25±5.14)mm and(51.74±4.62)mm,respectively)(t=4.743,4.490,both P<0.05),and the level of left ventricular ejection fraction((44.06±4.48)%)was higher than that in the control group((39.18±4.17)%)(t=6.859,P<0.05).After treatment,the levels of C-reactive protein,interleukin-6,N-terminal pro-B-type natriuretic peptide,soluble suppression of tumorigenicity 2,angiotensin Ⅱ,and endothelin-1 in the experimental group((5.78±0.92)mg/L,(10.26±2.52)pg/mL,(367.52±39.18)ng/L,(27.44±2.13)ng/mL,(60.35±5.28)ng/L,(37.36±4.68)ng/L,respectively)were all lower than those in the control group((8.03±1.26)mg/L,(15.34±3.37)pg/mL,(492.13±44.15)ng/L,(36.35±3.76)ng/mL,(73.26±6.55)ng/L,(44.25±4.13)ng/L,respectively)(t=12.406,10.385,18.160,17.737,13.200,9.496,all P<0.05).The incidence of adverse reac-tions was 5.41%(4/74)in the control group and 8.11%(6/74)in the experimental group,with no statistically significant differ-ence(χ2=0.429,P>0.05).Conclusion The addition of rBNP to conventional therapy combined with torasemide can effectively improve the overall clinical efficacy in patients with CHD and ALHF,enhance cardiac function,hemodynamic parameters,and neuroendocrine indicators,suppress inflammatory responses,and demonstrate a favorable safety profile.
