Drug resistance of levofloxacin eye drops combined with vancomycin in treatment of infectious endophthalmitis

Abstract:Objective To explore the efficacy of levofloxacin eye drops combined with vancomycin in the treatment of patients with infectious endophthalmitis and analyze the drug resistance.Methods A total of 325 patients with infectious endophthalmitis who were admitted to the Hebei Eye Hospital from January 2014 to December 2023 were enrolled in the study.According to different treatment methods,the patients were divided into the VAN group(n=117)and the LEV group(n=208).The VAN group was treated with vancomycin injection,while the LEV group was given vancomycin injection combined with levofloxacin eye drops.Both groups were treated for 14 days.The drug resistance,bacterial clearance rates,visual improvement,inflammatory factor levels and adverse reactions were compared between the two groups.Results The bacterial clearance rate was 86.84%(198/228)in the LEV group,higher than 71.20%(89/125)in the VAN group,and there was significant differ-ence(x2=12.995,P<0.05).After treatment,the visual acuity was(0.38±0.11)in the LEV group,better than(0.25±0.09)in the VAN group,and there was significant difference(t=1 1.517,P<0.05).The levels of serum high-sensitivity C-reactive protein and tumor necrosis factor-α of the LEV group were respectively(11.50±2.96)mg/L and(20.35±4.64)ng/mL,lower than(17.25±3.28)mg/L and(33.82±5.25)ng/mL of the VAN group,and there were significant differences(t=15.702,23.130,both P<0.05).The incidence of adverse reactions was 16.24%(19/117)in the VAN group and 17.31%(36/208)in the LEV group,and there was no significant difference(x2=0.061,P>0.05).The result of drug susceptibility testing showed that vancomycin did not have remark-able resistance to gram-positive bacteria.Conclusion Levofloxacin combined with vancomycin can effectively increase the bacterial clearance rate,improve visual acuity and alleviate inflammatory response,demonstrating favorable safety profile.
