Efficacy of irbesartan combined with metoprolol in patients with hypertension complicated by atrial fibrillation

Abstract:Objective To explore the clinical efficacy and safety of irbesartan in combination with metoprolol in the treatment of patients with hypertension complicated by atrial fibrillation.Methods A total of 125 patients with hypertension complicated by atrial fibrillation treated in the Department of Cardiovascular Medicine at Zhangjiagang Aoyang Hospital from January 2020 to June 2024 were included in this retrospective cohort study.Based on different treatment regimens,the patients were divided into the irbesartan group(n=63)and the metoprolol group(n=62).The metoprolol group was treated with metopr-olol monotherapy,while the irbesartan group was given irbesartan in combination with the treatment regimen of the metoprolol group.The treatment period for the irbesartan group and the metoprolol group was 6 months.The clinical efficacy,systolic blood pressure,diastolic blood pressure,frequency and duration of atrial fibrillation attacks,and changes in serum-related biomark-er levels of the irbesartan group and the metoprolol group before and after treatment were compared.Meanwhile,the occur-rence of adverse reactions was recorded.Results After 6 months of treatment,the total effective rate in the irbesartan group was significantly higher than that in the metoprolol group(92.06%(58/63)vs 77.42%(48/62),x2=5.199,P<0.05).The systolic blood pressure and diastolic blood pressure in the irbesartan group((108.33±8.87),(76.79±5.87)mmHg(1 mmHg=0.133 kPa))were lower than those in the metoprolol group((121.23±8.92),(84.51±6.03)mmHg)(t=8.107,7.253,both P<0.05).The atrial fibrillation attack frequency of the irbesartan group((10.45±2.58)times)was lower than that of the metoprolol group((12.89±2.45)times),and the attack duration in the irbesartan group((5.12±1.11)h)was shorter than that in the metoprolol group((6.51±1.23)h)(t=5.420,6.635,both P<0.05).The levels of C-reactive protein,tumor necrosis factor-α,and matrix metalloproteinase-2 in the irbesartan group((4.69±1.62)mg/L,(103.23±10.67)ng/L,(255.82±34.21)μg/L)were all lower than those in the metoprolol group((5.88±1.13)mg/L,(118.34±9.78)ng/L,(311.49±34.56)μg/L)(t=4.756,8.250,9.501,all P<0.05).The incidence of adverse reactions between the the irbesartan group and the metoprolol group was no statistically significant(11.11%(7/63)vs 16.13%(10/62),x2=0.670,P>0.05).Conclusion Irbesartan combined with metoprolol can improve the clinical efficacy in patients with hypertension complicated by atrial fibrillation,effectively control blood pressure levels,reduce the attack frequency of atrial fibril-lation,decrease the levels of relevant biomarkers in serum,and demonstrate a good safety profile during the treatment period.
