Efficacy of rosuvastatin at low to moderate doses combined with evolocumab in hyperlipidemia complicated with CHD

Abstract:Objective To investigate the efficacy and safety of rosuvastatin at low to moderate doses combined with evolocumab in patients with hyperlipidemia and coronary heart disease(CHD)after percutaneous coronary intervention(PCI).Methods A total of 86 patients with hyperlipidemia complicated by CHD who underwent PCI at the First Affiliated Hospital of Henan University of Science and Technology from May 2023 to December 2024 were selected and randomly divided into two groups using a random number table,with 43 cases in each group.The low-dose group received rosuvastatin(5 mg once daily)combined with evolocumab.The medium-dose group received rosuvastatin(10 mg once daily)combined with evolocumab.Both groups maintained treatment for 6 months.The clinical efficacy,lipid metabolism parameters,vascular endothelial function indicators,inflammatory factors,and the incidence of adverse reactions were compared between the two groups.Results After treatment,the total effective rate in the medium-dose group was 95.35%(41/43),higher than that in the low-dose group(81.40%(35/43))(x2=4.074,P<0.05).The levels of low-density lipoprotein cholesterol,triglyceride,and total cholesterol in the medi-um-dose group((1.17±0.34),(1.33±0.38),(3.31±0.40)mmol/L)lower than those in the low-dose group((1.46±0.37),(1.62±0.40),(3.65±0.65)mmol/L)(t=3.788,3.447,2.291,all P<0.05).The level of high-density lipoprotein cholesterol in the medium-dose group((1.54±0.27)mmol/L)was higher than that in the low-dose group((1.31±0.29)mmol/L)(t=3.806,P<0.05).The level of endothelin-1 in the medium-dose group((41.89±9.75)ng/L)was lower than that in the low-dose group((49.74±9.82)ng/L)(t=3.720,P<0.05),and the level of nitric oxide in the medium-dose group((71.25±12.86)μmol/L)was higher than that in the low-dose group((62.11±11.52)μmol/L)(t=3.471,P<0.05).The levels of high-sensitivity C-reactive protein and interleukin-6 in the medium-dose group((3.01±0.97)mg/L,(5.46±1.65)pg/L)were lower than those in the low-dose group((4.22±1.03)mg/L,(6.91±1.88)pg/L)(t=5.608,3.801,both P<0.05).The incidence of adverse reactions was 20.93%(9/43)in the medium-dose group and 11.63%(5/43)in the low-dose group,with no statistically significant difference between the two groups(x2=1.365,P>0.05).Conclusion Evolocumab combined with rosuvastatin at low to moderate doses demonstrates a favorable safety profile in treating hyperlipidemia with CHD after PCI.However,rosuvastatin at a dose of 10 mg demonstrates superior efficacy in improving clinical symptoms,regulating lipid metabolism,reducing inflammatory responses,and promoting vascular endothelial function repair.
