Application evaluation of remimazolam combined with remifentanil for non-intubated general anesthesia in ureteral stent removal

Abstract:Objective To investigate the safety and efficacy of remimazolam combined with remifentanil for non-intubated general anesthesia in ureteral stent removal(USR).Methods A total of 86 patients undergoing USR at Luoy-ang Central Hospital between February 2023 and August 2024 were selected and randomly assigned via computer-generated random numbers to the observation group(n=43)and the control group(n=43).The control group received propofol combined with remifentanil,while the observation group received remimazolam combined with remifentanil for general anesthesia main-tenance.Mean arterial pressure(MAP),heart rate(HR),and pulse oxygen saturation(SpO2)were compared between the two groups at different time points:before anesthesia(T1),during urethral insertion of the cystoscope(T2),during stent removal(T3),upon entering the post-anesthesia care unit(PACU)(T4),and upon leaving the PACU(T5).Anesthesia induction time,op-erative time,recovery time,PACU stay duration,and the incidence of adverse reactions were also compared between the two groups.Results The MAP in the observation group at T1,T2,T3,T4,and T5 were(91.49±7.04),(87.32±7.06),(85.21±6.89),(88.19±6.48),and(89.14±7.29)mmHg(1 mmHg=0.133 kPa),respectively,while those in the control group were(92.16±7.20),(84.26±6.77),(80.58±6.39),(83.07±6.44),and(88.70±6.24)mmHg,respectively.There were statistically significant time effects,interaction effects,and intergroup differences in MAP between the two groups of USR patients(Ftime=21.648,Finteraction=2.985,Fgroup=15.7 86,all P<0.05).The HR of the observation group at T1,T2,T3,T4,and T5 were(75.23±8.49),(73.89±8.08),(71.28±5.25),(72.53±6.44),and(73.70±6.18)beats/min,respectively,while those of the control group were(75.34±9.63),(70.33±8.18),(64.16±5.58),(68.35±5.47),and(72.26±6.54)beats/min,respectively.There were statistically sig-nificant time effects,interaction effects,and intergroup differences in HR between the two groups of USR patients(Ftime=13.361,Finteraction=3.200,Fgroup=23.378,all P<0.05).The SpO2 in the observation group at T1,T2,T3,T4,and T5 were(98.54±6.33),(98.49±5.34),(97.89±6.02),(98.62±5.67),and(99.13±3.60)%,respectively,while those in the control group were(97.21±6.12),(96.11±5.49),(95.09±6.08),(96.79±5.78),and(98.73±4.10)%,respectively.There was no statistically significant time effect or interaction effect on SpO2 in the two groups of USR patients(Ftime=2.253,Finteraction=0.625,both P>0.05),while the between-group effect showed statistical significance(Fgroup=10.351,P<0.05).Awakening time((3.15±1.21)min vs(3.86±1.52)min)and PACU stay time((30.12±6.03)min vs(32.87±6.09)min)were both shorter in the observation group than in the control group(t=2.396,2.104,both P<0.05).The incidence of adverse reactions was 4.65%(2/43)in the observation group and 11.63%(5/43)in the control group,with no statistically significant difference(Fisher's exact probability test,P=0.438).Conclusion The use of remimazolam combined with remifentanil for general anesthesia maintenance in patients undergoing USR provides stable hemo-dynamics,shortens recovery and PACU stay times,and does not increase the incidence of adverse reactions.
