Serum FAK and FABP4 as predictors of cardiotoxicity in HER2-positive breast cancer patients receiving postoperative trastuzumab-based targeted therapy

Abstract:Objective To investigate the clinical value of serum focal adhesion kinase(FAK)and fatty acid-binding protein 4(FABP4)in predicting cardiotoxicity during postoperative trastuzumab-based targeted therapy in patients with human epidermal growth factor receptor 2(HER2)-positive breast cancer.Methods A retrospective inclusion of 136 female patients with HER2-positive breast cancer who underwent postoperative trastuzumab-based targeted therapy at Suzhou Integrated Tra-ditional Chinese and Western Medicine Hospital between May 2020 and May 2023 were selected.They were divided into the cardiotoxicity group(n=36)and the non-cardiotoxicity group(n=100)based on the occurrence of cardiotoxicity during targeted therapy.Serum levels of FAK and FABP4 were compared among the groups.Receiver operating characteristic curve analysis was used to assess predictive value,and multivariate logistic regression was employed to analyze influencing factors for cardio-toxicity.Results The cardiotoxicity group had significantly higher serum levels of FAK and FABP4 compared to the non-car-diotoxicity group((48.55±4.12)μg/L vs(34.38±3.08)μg/L,(47.47±1.12)ng/mL vs(30.36±3.62)ng/mL)(t=21.552,27.825,both P<0.001).The area under the curve(AUC)for the combined prediction of cardiotoxicity by serum FAK and FABP4 was 0.932(95%CI:0.880-0.963),which was significantly higher than that for FAK alone(0.810(95%CI:0.751-0.856))and FABP4 alone(0.752(95%CI:0.689-0.792))(Z=10.149,9.326,respectively,both P<0.001).The cardiotoxicity group also had higher serum cardiac troponin T and B-type natriuretic peptide levels and lower left ventricular ejection fraction(LVEF)than the non-car-diotoxicity group(all P<0.05).Multivariate Logistic analysis showed that LVEF≤47.65%(OR=3.755,95%CI:2.766-5.098),FAK≥39.35 μg/L(OR=3.337,95%CI:1.842-6.043),and FABP4 ≥ 37.65 ng/mL(OR=3.074,95%CI:2.025-4.667)were in-dependent influencing factors for cardiotoxicity following postoperative targeted therapy in breast cancer patients(all P<0.05).Conclusion Serum FAK and FABP4 can effectively predict the risk of cardiotoxicity during postoperative trastuzumab-based targeted therapy in HER2-positive breast cancer patients,and their combined prediction offers superior clinical value.
