Efficacy of different doses of biapenem combined with ulinastatin on treatment of patients with severe pneumonia

Abstract:Objective To explore the efficacy of different doses of biapenem combined with ulinastatin on treat-ment of patients with severe pneumonia and the impact on symptomatic indexes and pulmonary function.Methods A total of 82 patients with severe pneumonia treated in the First People's Hospital of Zhumadian from February 2021 to December 2024 were retrospectively enrolled in the study and divided into the conventional group with 39 cases and the high-dose group with 43 cases according to the different doses of biapenem.The conventional group was treated with the conventional dose of biapenem(0.3 g every time,twice a day)combined with ulinastatin,while the high-dose group was given the high dose of biapenem(0.6 g every time,twice a day)combined with ulinastatin.Both groups were treated for 2 consecutive weeks.The clinical efficacy,disease severity(CURB-65),symptom improvement,pulmonary function indexes(forced expiratory volume in the first second(FEV1),forced vital capacity(FVC)and peak expiratory flow(PEF)),levels of inflammatory factors(C-reactive protein(CRP)and procalcitonin(PCT)),and adverse reactions were compared between the two groups.Results The total effective rate of the high-dose group was 88.37%(38/43)after treatment,higher than 69.23%(27/39)of the conventional group(χ2=4.560,P<0.05);the CURB-65 score of the high-dose group was(1.04±0.21)points,lower than(1.25±0.35)points of the conventional group(t=3.330,P<0.05).The time of disappearance of fever,cough and lung rales of the high-dose group was respectively(6.53±1.41)d,(7.61±1.71)d and(8.57±1.96)d,shorter than(7.35±1.69)d,(8.53±1.85)d and(9.96±2.08)d of the conventional group(t=2.393,2.340,3.115,all P<0.05).The FEV1,FVC and PEF of the two groups increased after treatment,and the FEV1,FVC and PEF of the high-dose group were respectively(1.91±0.45)L,(2.47±0.52)L and(2.58±0.53)L/s,higher than(1.67±0.34)L,(2.21±0.43)L and(2.28±0.46)L/s of the conventional group(t=2.703,2.453,2.724,all P<0.05).The levels of CRP and PCT of the two groups were reduced after treatment,and the levels of CRP and PCT of the high-dose group were respectively(81.56±15.41)mg/L and(2.13±0.45)ng/mL,lower than(91.47±17.13)mg/L and(2.85±0.52)ng/mL of the conventional group(t=2.758,6.720,both P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(20.93%(9/43)vs 15.38%(6/39))(χ2=0.421,P>0.05).Conclusion The high dose of biapenem combined with ulinastatin shows more remarkable clinical effect in the treatment of the patients with severe pneumonia.It can not only alleviate clinical symptoms,reduce the levels of inflammatory responses,but also promote the recovery of pulmonary function,demon-strating a favorable safety profile.
