Comparative study of the efficacy of conbercept and dexamethasone in the treatment of patients with diabetic macular edema

Abstract：Objective To compare the clinical efficacy and safety of intravitreal conbercept versus dexamethasone implant in the treatment of diabetic macular edema(DME)after pars plana vitrectomy(PPV).Methods A retrospective analy-sis was conducted on 106 DME patients who underwent PPV at Nantong First People's Hospital from January 2021 to Decem-ber 2024.Based on postoperative medication regimens,patients were divided into two groups:the dexamethasone group(n=50,treated with dexamethasone intravitreal implant),and the conbercept group(n=56,treated with intravitreal conbercept injection).The foveal avascular zone area,best-corrected visual acuity(BCVA),intraocular pressure(IOP),central retinal thickness(CRT),and the incidence of adverse reactions were compared between the two groups before surgery and at 1,3,and 6 months after sur-gery.Results Postoperative BCVA decreased in both groups compared to preoperative levels.At 6 months postoperatively,the dexamethasone group showed better BCVA((0.48±0.15)logMAR)than the conbercept group((0.54±0.14)logMAR)(t=2.130,P＜0.05).CRT decreased postoperatively in both groups.At 6 months postoperatively,the dexamethasone group exhibited a low-er CRT((248.33±38.42)µm)than the conbercept group((264.45±36.57)µm)(t=2.212,P＜0.05).There were no statistically significant differences in IOP between the two groups before surgery and at 1,3,and 6 months after surgery(t=0.341,1.450,0.719,0.527,all P＞0.05).The incidence of adverse reactions was 16.00%(8/56)in the dexamethasone group and 7.14%(4/56)in the conbercept group,with no statistically significant difference(χ2=2.046,P＞0.05).Conclusion For DME patients after PPV,the dexamethasone intravitreal implant demonstrates superior efficacy to ranibizumab in promoting macular anatomical restoration and visual acuity improvement,without increasing IOP or the risk of complications.
