Efficacy of venetoclax combined with decitabine in elderly AML patients with TET2 gene mutation

Abstract:Objective To investigate the efficacy of disease control and adverse reactions of venetoclax combined with decitabine in elderly acute myeloid leukemia(AML)patients with tet methylcytosine dioxygenase 2(TET2)gene mutation.Methods A total of 60 elderly patients with TET2-mutated AML who received induction therapy with venetoclax combined with decitabine at the Third Affiliated Hospital of Xinxiang Medical University between January 2021 and November 2023 were retrospectively analyzed.Based on their physical fitness and disease status,they were divided into the intensive chemo-therapy-ineligible group(n=35)and the relapsed/refractory group(n=25).Clinical efficacy,overall survival(OS)and adverse reactions were analyzed.Results After one treatment course,the objective response rate was 77.14%(27/35)in the intensive chemotherapy-ineligible group and 68.00%(17/25)in the relapsed/refractory group,with no statistically significant difference(χ2=0.623,P>0.05).The composite complete remission rate was 48.57%(17/35)in the intensive chemotherapy-ineligible group and 32.00%(8/25)in the relapsed/refractory group,with no statistically significant difference between the two groups(χ2=1.648,P>0.05).By the end of follow-up,a total of 39 patients survived.In the non-intensive chemotherapy group,25 patients survived and 10 died,with a 1-year OS rate of 71.43%(25/35).In the relapsed/refractory group,14 patients survived and 11 died,with a 1-year OS rate of 56.00%(14/25),with no statistically significant difference between the two groups(Log-rank χ2=1.475,P=0.225).All patients experienced grade 3-4 hematological adverse reactions associated with myelosuppression,and other com-mon adverse effects included fever(75.00%,45/60),pulmonary infection(51.67%,31/60),sepsis(11.67%,7/60),and urinary tract infection(10.00%,6/60).Conclusion The combination of venetoclax and decitabine in treating elderly AML patients with TET2 gene mutations can alleviate bone marrow cell morphology,stabilize overall survival,and does not increase adverse reac-tions,demonstrating comparable efficacy for both patient subgroups.
