Effect of live Bacillus licheniformis capsules on intestinal function in patients with Crohn's disease

Abstract:Objective To explore the effect of live Bacillus licheniformis capsules as an adjunctive therapy on intestinal function in patients with Crohn's disease.Methods A total of 107 patients with Crohn's disease treated at Hai'an People's Hospital from March 2021 to March 2025 were selected and divided into two groups based on their treatment regi-mens:the Bacillus licheniformis group(57 cases)and the mesalazine group(50 cases).The mesalazine group received mesal-azine enteric-coated tablets,while the Bacillus licheniformis group received live Bacillus licheniformis capsules in addition to mesalazine enteric-coated tablets as adjuvant therapy.The clinical efficacy,Crohn's Disease Activity Index(CDAI)scores,Inflammatory Bowel Disease Questionnaire(IBDQ)scores,intestinal barrier function indicators(diamine oxidase,endotox-in,D-lactate),intestinal microbiota(Bifidobacterium,Lactobacillus,Escherichia coli),inflammatory markers(tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),C-reactive protein(CRP),erythrocyte sedimentation rate)levels,and the incidence of adverse reactions between the two groups were compared.Results The total effective rate in the Bacillus licheniformis group was 92.98%(53/57),higher than 78.00%(39/50)in the mesalazine group(χ2=4.960,P=0.026).After treatment,the CDAI score in the Bacillus licheniformis group((95.05±20.33)points)was lower than that in the mesalazine group((121.34±26.26)points),while the IBDQ score in the Bacillus licheniformis group((167.24±40.88)points)was higher than that in the mesala-zine group((134.72±35.29)points)(t=5.827,4.374,both P<0.05).After treatment,the levels of diamine oxidase,endotoxin,and D-lactate in the Bacillus licheniformis group((4.17±1.07)U/mL,(0.06±0.01)EU/mL,(3.14±0.59)mmol/L,respectively)were all lower than those in the mesalazine group((5.26±1.36)U/mL,(0.09±0.02)EU/mL,(4.66±0.85)mmol/L,respectively)(t=4.634,9.994,10.849,all P<0.05).After treatment,the counts of Bifidobacterium and Lactobacillus in the Bacillus licheni-formis group((9.24±1.88)lg CFU/g,(7.99±1.24)lg CFU/g,respectively)were higher than those in the mesalazine group((7.62±1.43)lg CFU/g,(6.23±0.81)lg CFU/g,respectively),while the count of Escherichia coli((8.02±1.17)lg CFU/g)was lower than that in the mesalazine group((9.22±1.35)lg CFU/g)(t=4.962,8.559,4.926,all P<0.05).After treatment,the levels of CRP,IL-6,TNF-α,and erythrocyte sedimentation rate in the Bacillus licheniformis group((10.51±3.34)mg/L,(26.25±7.49)pg/mL,(18.54±5.37)pg/mL,(14.71±3.78)mm/h,respectively)were all lower than those in the mesalazine group((14.89±4.41)mg/L,(38.06±9.28)pg/mL,(25.51±6.05)pg/mL,(18.02±4.25)mm/h,respectively)(t=5.832,7.279,6.314,4.264,all P<0.05).The incidence of adverse reactions was 8.77%(5/57)in the Bacillus licheniformis group and 12.00%(6/50)in the mesalazine group,with no statistically significant difference(χ2=0.301,P=0.583).Conclusion Adjuvant therapy with live Bacillus licheniformis capsules demonstrates definite efficacy in patients with Crohn's disease.It not only alleviates clinical symptoms by enhancing intestinal barrier function and inhibiting inflammatory responses but also effectively improves patients'quality of life,with a fa-vorable safety profile.
