Efficacy of vericiguat combined with metoprolol in elderly patients with coronary heart disease and HFrEF

Abstract：Objective To explore the clinical efficacy of vericiguat combined with metoprolol in elderly patients with coronary heart disease(CHD)complicated with heart failure with reduced ejection fraction(HFrEF).Methods A total of 84 elderly patients with CHD and HFrEF admitted to Puyang Oilfield General Hospital from June 2023 to December 2024 were retrospectively selected and divided into the metoprolol group and the combination therapy group,with 42 cases in each group.The metoprolol group received foundational therapy for CHD and HFrEF plus metoprolol,while the combination therapy group received foundational therapy for CHD and HFrEF plus the combination of metoprolol and vericiguat.Both groups were treated for 3 months.Clinical efficacy,6 minute walk test distance(6-MWD),cardiac function indicators,and the incidence of adverse reactions were compared between the two groups.Results After treatment,the total effective rate in the combination therapy group was 90.48%(38/42),significantly higher than 73.81%(31/42)in the metoprolol group(χ2=3.977,P＜0.05),and the 6-MWD in the combination therapy group was(398.83±15.06)m,significantly longer than(375.52±15.29)m in the metop-rolol group(t=7.039,P＜0.05).After treatment,the left ventricular end-systolic diameter,left ventricular end-diastolic diameter,and N-terminal pro-B-type natriuretic peptide levels in the combination therapy group were significantly lower than those in the metoprolol group((40.82±1.26)mm vs(44.63±2.02)mm,(53.52±2.11)mm vs(56.31±2.16)mm,(1 641.83±107.28)pg/mL vs(1 951.72±111.51)pg/mL,respectively)(t=10.371,5.988,12.979,all P＜0.05),and the level of left ventricular ejection frac-tion was(42.15±2.71)%in the combination therapy group,higher than that in the metoprolol group((40.02±2.26)%)(t=3.912,P＜0.05).The incidence of adverse reactions was 11.90%(5/42)in the metoprolol group and 14.29%(6/42)in the combination therapy group,with no statistically significant difference(χ2=0.105,P＞0.05).Conclusion In the treatment of elderly patients with CHD and HFrEF,the combination of vericiguat and metoprolol can enhance clinical efficacy,improve cardiac function,en-hance quality of life,while demonstrating a favorable safety profile.
