Efficacy of captopril in elderly patients with cor pulmonale and right heart failure

Abstract:Objective To investigate the clinical efficacy and impact on cardiopulmonary function of captopril com-bined with furosemide and isosorbide dinitrate in elderly patients with cor pulmonale and right heart failure.Methods A total of 108 elderly patients with cor pulmonale and right heart failure admitted to Jiangsu Province Hospital of Chinese Medicine from May 2022 to March 2025 were retrospectively selected as the research subjects and divided into the combination group and the non-combination group based on their treatment regimen.The non-combination group received furosemide and isosor-bide dinitrate,while the combination group received captopril in addition to furosemide and isosorbide dinitrate.Therapeutic outcomes,changes in cardiopulmonary function indicators,and the incidence of adverse reactions were compared between the two groups.Results The total effective rate was 94.44%(51/54)in the combination group,significantly higher than 79.63%(43/54)in the non-combination group(χ2=5.252,P<0.05).After treatment,pulmonary artery systolic pressure(PASP),right ventricular myocardial performance index(RVMPI),and B-type natriuretic peptide(BNP)levels decreased in both groups com-pared to pre-treatment levels.The levels of PASP,RVMPI,and BNP in the combination group((45.13±5.09)mmHg(1 mmHg=0.133 kPa),(0.45±0.07),and(185.37±23.12)pg/mL,respectively)were lower than those in the non-combination group((47.15±5.04)mmHg,(0.51±0.08),and(226.39±25.52)pg/mL,respectively)(t=2.072,4.148,8.754,all P<0.05).The ratio of early to late(atrial)diastolic filling velocities(E/A ratio)and tricuspid annular plane systolic excursion(TAPSE)increased in both groups post-treatment.However,there was no statistically significant difference in the E/A ratio between the combination group and the non-combination group((0.86±0.06)vs(0.88±0.07))(P>0.05).The TAPSE in the combination group((15.83±1.51)mm)was higher than that in the non-combination group((15.16±1.73)mm),and the difference was statistically significant(t=2.144,P<0.05).After treatment,the levels of forced vital capacity(FVC),forced expiratory volume in one second(FEV1),and FEV1/FVC ratio in the combination group((2.53±0.41)L,(1.78±0.27)L,(70.36±11.34)%,respectively)were higher than those in the non-combination group((2.24±0.30)L,(1.43±0.42)L,(63.84±10.16)%,respectively)(t=4.195,5.151,3.147,all P<0.05).The incidence of adverse reactions in the combination group was lower than that in the non-combination group(3.70%vs 14.81%),and the difference was statistically significant(χ2=3.967,P<0.05).Conclusion The combination therapy of capto-pril,furosemide,and isosorbide dinitrate demonstrates significant efficacy,effectively improving cardiopulmonary function and reducing the incidence of adverse reactions in elderly patients with cor pulmonale and right heart failure.
