Efficacy of Shenfu Decoction combined with Danshen Drink and recombinant human brain natriuretic peptide in HFrEF patients with yang deficiency and water flooding pattern

Abstract：Objective To investigate the clinical efficacy of Shenfu Decoction combined with Danshen Drink and recombinant human brain natriuretic peptide(rhBNP)in the treatment of heart failure with reduced ejection fraction(HFrEF)patients presenting with yang deficiency and water flooding pattern.Methods A total of 120 HFrEF patients with yang defi-ciency and water flooding pattern treated at Taihe County Hospital of Traditional Chinese Medicine(TCM)from January 2022 to January 2025 were selected and randomly divided into the combination group and the control group,with 60 patients in each group.Both groups received conventional anti-heart failure therapy.The control group received intravenous infusion of rhBNP for 3 consecutive days,while the combination group received additional treatment with Shenfu Decoction combined with Dan-shen Drink on the basis of the control group's regimen for 2 consecutive months.Clinical efficacy and adverse reactions were compared between the two groups.TCM syndrome scores,6-minute walk test distance(6MWD),Minnesota Living with Heart Failure Questionnaire(MLHFQ)scores,cardiac function,and laboratory indicators were compared before and after treatment.Results After treatment,the combination group showed lower TCM syndrome scores,MLHFQ scores,left ventricular end-di-astolic diameter,left ventricular end-systolic diameter,B-typenatriuretic peptide,and troponinⅠlevels((9.31±1.76)points,(39.32±5.47)points,(57.43±5.38)mm,(45.64±4.16)mm,(273.56±37.25)pg/mL,(1.76±0.52)ng/mL,respectively)compared to the control group((14.85±1.87)points,(46.73±5.97)points,(60.82±5.74)mm,(48.92±4.35)mm,(356.85±43.63)pg/mL,(2.14±0.61)ng/mL,respectively)(t=16.711,7.089,3.338,4.221,11.246,3.672,all P＜0.05).Additionally,the 6MWD and left ventricular ejection fraction in the combination group((384.72±33.47)m,(41.86±5.34)%,respectively)were significantly high-er than those in the control group((321.69±30.51)m,(37.54±4.92)%,respectively)(t=10.780,4.609,both P＜0.05).The total effective rate in the combination group was 91.67%(55/60),higher than that in the control group(76.67%(46/60))(χ2=5.065,P＜0.05).There was no difference in adverse reactions between the two groups(6.67%(4/60)vs 10.00%(6/60))(χ2=0.436,P=0.509).Conclusion The combination of Shenfu Decoction,Danshen Drink,and rhBNP can significantly alleviate clinical symptoms and myocardial injury,improve cardiac function,enhance exercise tolerance and quality of life in HFrEF patients with yang deficiency and water flooding pattern,demonstrating favorable safety.
