Clinical evaluation of thymopentin combined with levofloxacin-containing anti-tuberculosis regimen in the treatment of isoniazid-resistant pulmonary tuberculosis

Abstract:Objective To explore the effect of thymopentin combined with levofloxacin-containing anti-tuberculosis regimen on sputum negative conversion rate and immune function in patients with isoniazid-resistant pulmonary tuberculosis.Methods A total of 102 isoniazid-resistant tuberculosis patients admitted to the Third Hospital of Cangzhou and Hebei Chest Hospital from January 2022 to December 2024 were selected and randomly divided into the control group(n=51,treated with a levofloxacin-containing anti-tuberculosis regimen)and the observation group(n=51,treated with thymopentin combined with a levofloxacin-containing anti-tuberculosis regimen) using a random number table.The sputum negative conversion rate,immune indicators(T lymphocyte subsets including CD3+,CD4+,CD8+),levels of inflammatory indicators(C-reactive protein,procalci-tonin,erythrocyte sedimentation rate)and the incidence of adverse reactions before and after treatment were compared between the observation group and the control group.Results The sputum negative conversion rates in the observation group after 2,4 and 6 months of treatment were 72.55%(37/51),84.31%(43/51)and 94.12%(48/51),respectively,higher than 52.94%(27/51),66.67%(34/51),and 78.43%(40/51)in the control group(χ²=4.194,4.292,5.299,all P<0.05).After treatment,the levels of CD3+T and CD4+T lymphocytes in the observation group and the control group increased,while the level of CD8+T lympho-cytes decreased.The levels of CD3+T and CD4+T lymphocytes in the observation group((68.62±6.73)%and(39.16±3.79)%,respectively)were higher than those in the control group((61.57±6.24)%and(36.23±3.71)%,respectively),and the level of CD8+T lymphocytes in the observation group((22.15±2.42)%)was lower than that in the control group((24.37±2.59)%)(t=5.486,3.945,4.473,all P<0.05).After treatment,the levels of serum C-reactive protein,procalcitonin and erythrocyte sedimenta-tion rate decreased in the observation group and the control group,and the above indicators in the observation group((13.28±1.66)mg/L,(0.43±0.07)ng/mL,(25.47±5.51)mm/h)were lower than those in the control group((16.17±1.89)mg/L,(0.60±0.10)ng/mL,(31.41±5.97)mm/h)(t=8.205,9.946,5.222,all P<0.05).The incidence rates of adverse reactions in the observation group and the control group were 19.61%(10/51)and 13.73%(7/51),respectively,and the difference between the two groups was not statistically significant(χ²=0.635,P>0.05).Conclusion Thymopentin combined with levofloxacin-containing anti-tu-berculosis regimen can increase the sputum negative conversion rate,improve immune function,reduce inflammatory levels in patients with isoniazid-resistant pulmonary tuberculosis,and demonstrate acceptable safety.
