Efficacy of Qinggan Dingxuan Pills combined with betahistine in the treatment of posterior circulation ischemic vertigo

Abstract:Objective To explore the efficacy of Qinggan Dingxuan Pills combined with betahistine in the treat-ment of posterior circulation ischemic vertigo(PCIV)(Liver Yang hyperactivity syndrome)and its influence on hemodynamics.Methods A total of 138 patients with PCIV(Liver Yang hyperactivity syndrome)who received diagnosis and treatment at Anyang Hospital of Traditional Chinese Medicine(TCM)from January 2023 to September 2024 were enrolled in this prospec-tive cohort study.The patients were randomly assigned to the observation group and the control group using a random number table method,with 69 cases in each group.Upon admission,all patients immediately received standardized steady-state reg-ulation measures,including blood pressure,blood glucose,and lipid levels.The control group was treated with intravenous infusion of betahistine followed by oral betahistine tablets on this basis.The observation group received Qinggan Dingxuan Pills in addition to the control group's treatment regimen,with a course of treatment lasting 4 weeks.The clinical efficacy,inci-dence of adverse reactions,changes in TCM syndrome scores,Dizziness Handicap Inventory(DHI)scores,and hemodynamic indicators before and after treatment were compared between the control group and the observation group.Results After the treatment,the total effective rate of the observation group was higher than that of the control group(91.30%(63/69)vs 78.26%(54/69)),with a statistically significant difference(χ2=4.549,P<0.05).The integral scores of primary symptoms(dizziness,tin-nitus,and distension in the head)and secondary symptoms(red face and eyes,bitter taste in the mouth,and limb numbness)in the observation group were significantly lower than those in the control group((1.28±0.37)points vs(2.46±0.36)points,(1.34±0.33)points vs(2.61±0.32)points,(1.31±0.35)points vs(2.51±0.32)points,(0.67±0.22)points vs(1.46±0.23)points,(0.72±0.26)points vs(1.39±0.24)points,(0.65±0.27)points vs(1.37±0.25)points),with statistically significant differences(t=18.987,22.950,21.019,20.618,15.729,16.254,all P<0.05).The score of the DHI in the observation group was lower than that in the control group((13.12±1.15)points vs(17.32±1.24)points),with a statistically significant difference(t=20.629,P<0.05).The average levels of the peak systolic velocity,end-diastolic velocity and mean velocity of bilateral vertebral ar-teries and basilar arteries in the observation group were higher than those in the control group((48.88±7.09)cm/s vs(44.73±6.89)cm/s,(17.57±2.23)cm/s vs(15.67±2.19)cm/s,(29.79±5.12)cm/s vs(26.89±4.23)cm/s),with statistically significant dif-ferences(t=3.487,5.050,3.627,all P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the observation group and the control group in patients with PCIV(8.70%(6/69)vs 10.14%(7/69)),with no statistically significant difference(χ2=0.085,P>0.05).Conclusion Qinggan Dingxuan Pills can effectively improve the manifestations of TCM syndromes,vertigo symptoms,and posterior circulation hemodynamic indicators of patients with PCIV(Liver Yang hyper-activity syndrome).It demonstrates good safety in use and high clinical application value.
