Efficacy of denosumab in elderly patients with osteoporotic thoracolumbar vertebral fractures following percutaneous vertebro plasty

Abstract:Objective To observe the efficacy of denosumab as an adjunctive treatment following percutaneous vertebro plasty(PVP)in elderly patients with osteoporotic thoracolumbar vertebral fractures.Methods A total of 100 elderly patients with osteoporotic thoracolumbar vertebral fractures who received targeted treatment at Union Hospital,Tongji Medical College,Huazhong University of Science and Technology from January 2023 to January 2024 were divided into two groups according to different treatment methods.Both groups of patients were admitted and received PVP.The control group was treat-ed with conventional calcium supplements after the operation,while the study group was treated with denosumab.The clinical efficacy,bone mineral density(BMD),anterior vertebral height loss rate,pain degree,lumbar function,and the incidence of refracture and adverse reactions were compared between the two groups.Results The total effective rate in the study group was 96.00%(48/50),higher than 84.00%(42/50)in the control group(χ2=4.000,P=0.046).After treatment,the bone mineral density of patients in the study group and the control group((0.88±0.20)g/m² vs(0.70±0.18)g/m²,respectively)was higher than before treatment((0.55±0.15)g/m² vs(0.53±0.14)g/m²,respectively),with the study group showing significantly higher values than the control group(t=4.730,P<0.05).The anterior vertebral height loss rate((24.89±2.56)%vs(28.37±2.43)%,respectively)was lower than before treatment((31.12±2.54)%vs(31.27±2.48)%,respectively),with the study group demon-strating a significantly lower rate than the control group(t=6.972,P<0.05).After treatment,the Visual Analogue Scale score for pain and the Oswestry Disability Index score of the patients in the study group and the control group were(2.10±0.64)points,(20.16±4.02)points,(3.88±0.96)points,and(25.66±4.63)points,respectively,lower than those before treatment((8.01±1.66)points,(36.20±5.62)points,(7.96±1.60)points,(36.15±5.33)points,respectively),and the above indicators in study group was lower than those in the control group,and the differences were statistically significant(t=-10.909,-6.343,both P<0.05).The incidence of adverse reactions in the study group after treatment was 4.00%(2/50),and there was no significant difference with 10.00%(5/50)in the control group(χ2=0.614,P=0.433).The incidence of refracture in the study group was 6.00%(3/50),lower than 26.00%(13/50)in the control group(χ2=7.440,P=0.006).Conclusion The effect of PVP followed by denosumab in the treatment of elderly patients with osteoporotic thoracolumbar fractures is better,as it can increase bone mineral density,reduce the severity of bone pain,improve lumbar function,and decrease the incidence of refracture.
