Efficacy of saxagliptin-metformin sustained-release tablets combined with dapagliflozin in treating patients with type 2 diabetes mellitus and obesity

Abstract:Objective To investigate the clinical efficacy of saxagliptin-metformin sustained-release tablets com-bined with dapagliflozin in the treatment of type 2 diabetes mellitus(T2DM)and obesity.Methods A total of 104 patients with T2DM and obesity who were treated in Zhongshan Chenxinghai Hospital of Integrated Traditional Chinese and Western Medicine from June 2023 to December 2024 was retrospectively selected and divided into the observation group and the control group based on different treatment regimens,52 patients in each group.The control group was treated with saxagliptin-met-formin sustained-release tablets,while the observation group received saxagliptin-metformin sustained-release tablets combined with dapagliflozin.The clinical efficacy,the levels of blood glucose-related indicators,blood lipid-related indicators,pancreatic islet function,inflammatory factors and adipokines,body mass index,and the incidence of adverse reactions were compared between the two groups.Results The total effective rate in the observation group(94.23%(49/52))was higher than that in the control group(80.78%(42/52))(χ2=4.308,P=0.038).After treatment,the levels of the fasting blood glucose,2-hour post-prandial glucose and glycated hemoglobin in the observation group((6.28±0.32)mmol/L,(8.11±0.37)mmol/L,(6.72±0.32)%,respectively)were lower than those in the control group((7.04±0.37)mmol/L,(9.27±0.54)mmol/L,(7.63±0.56)%,respectively)(t=11.203,12.779,10.174,all P<0.05).After treatment,the total cholesterol,triglycerides,and low-density lipoprotein choles-terol levels in the observation group((3.22±0.36)mmol/L,(1.32±0.30)mmol/L,(2.02±0.36)mmol/L,respectively)were lower than those in the control group((3.97±0.54)mmol/L,(1.75±0.36)mmol/L,(2.43±0.41)mmol/L,respectively),while the high-den-sity lipoprotein cholesterol level in the observation group((1.37±0.22)mmol/L)was higher than that in the control group((1.13±0.19)mmol/L)(t=8.333,6.617,5.419,5.954,all P<0.05).After treatment,the fasting insulin,fasting C-peptide and insulin resis-tance index of the observation group((9.33±1.71)μIU/mL,(1.76±0.21)ng/mL,(3.82±0.91),respectively)were lower than those of the control group((10.18±1.89)μIU/mL,(1.95±0.28)ng/mL,(4.27±1.05),respectively),while the insulin β-cell function in-dex of the observation group((65.96±14.08)%)was higher than that of the control group((58.32±13.14)%)(t=2.405,3.915,2.335,2.861,all P<0.05).After treatment,the levels of C-reactive protein and interleukin-6 in the observation group((5.83±0.42)mg/L,(11.35±2.08)pg/mL,respectively)were lower than those in the control group((6.62±0.61)mg/L,(14.47±2.21)pg/mL,respec-tively)(t=7.692,7.413,both P<0.05).After treatment,the adiponectin level in the observation group((6.21±1.14)ng/mL)was higher than that in the control group((4.92±1.06)ng/mL),and the leptin level in the observation group((6.53±1.51)pg/mL)was lower than that in the control group((8.72±1.68)pg/mL)(t=6.055,6.991,both P<0.05).After treatment,the body mass index in the observation group((26.34±1.08)kg/m2)was lower than that in the control group((27.57±1.26)kg/m2)(t=5.345,P<0.05).The difference in adverse reaction rates between the two groups was not statistically significant(11.54%(6/52)vs 7.69%(4/52))(χ2=0.443,P=0.506).Conclusion The combination of saxagliptin-metformin sustained-release tablets and dapagliflozin in the treatment of T2DM and obesity has shown significant efficacy.It can effectively control patients'blood glucose and lipid levels,improve insulin resistance,reduce inflammatory responses,lower body mass index,and demonstrate a high safety profile.
