Efficacy of bisoprolol combined with isosorbide mononitrate in the treatment of angina pectoris complicated with chronic heart failure

Abstract:Objective To investigate the efficacy and safety of the combined regimen of bisoprolol and isosorbide mononitrate in patients with angina pectoris and comorbid chronic heart failure(CHF).Methods A randomized controlled trial design was adopted.A total of 100 patients diagnosed with angina pectoris complicated with CHF admitted in the First Affiliated Hospital of Xi'an Jiaotong University between February 2023 and February 2025 were enrolled in this study and divided into the control group(n=50)and the observation group(n=50)using a random number table method.The control group was given iso-sorbide mononitrate,while the observation group was given bisoprolol and isosorbide mononitrate.After 12 weeks of treatment,the clinical efficacy,changes in left ventricular ejection fraction(LVEF)and left ventricular end-diastolic diameter(LVEDD),an-gina pectoris attacks before and after treatment,and the incidence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 96.00%(48/50),higher than that of the control group(84.00%(42/50)),and the difference was statistically significant(χ2=4.000,P<0.05).After treatment,the LVEF of both groups increased compared with that before treatment,and the LVEF of the observation group((51.46±4.22)%)was higher than that of the con-trol group((48.31±3.69)%)(t=3.936,P<0.05).After treatment,the LVEDD and Tei index(myocardial performance index)de-creased,and the LVEDD and Tei index in the observation group((50.09±4.25)mm and(0.42±0.08),respectively)were lower than those in the control group((52.35±4.58)mm and(0.46±0.09))(t=2.558,2.349,both P<0.05).After treatment,the frequency and duration of angina pectoris attacks in both groups improved compared with those before treatment,and the frequency and duration of angina pectoris attacks in the observation group((1.88±0.62)times/week,(2.36±0.84)min)were lower than those in the control group((3.21±0.97)times/week,(3.87±1.05)min)(t=5.630,7.941,both P<0.05).The difference in the incidence of adverse reactions between the control group and the observation group was not statistically significant(16.00%(8/50)vs 20.00%(10/50))(χ2=0.271,P>0.05).Conclusion The combination of bisoprolol and isosorbide mononitrate can significantly improve the cardiac function level of patients with angina pectoris combined with CHF,effectively control the occurrence of angina pec-toris symptoms,improve exercise tolerance,and have good safety.
