Clinical evaluation of tirofiban and oxiracetam in treatment of patients with acute progressive ischemic stroke

Abstract:Objective To investigate the combined effects of tirofiban and oxiracetam on neurological function,cog-nitive function,regulation of complement proteins C3 and C4,and clinical efficacy in patients with acute progressive ischemic stroke(PIS).Methods A total of 114 PIS patients admitted to the Department of Neurology of CangZhou Hospital of Integrated Traditional Chinese and Western Medicine in Hebei Province from July 2021 to July 2023 were selected as study subjects.They were randomly assigned to the control group and the observation group using a random number table method,with 57 cases in each group.The control group received conventional dual antiplatelet and anticoagulant therapy combined with tirofiban,while the observation group received conventional dual antiplatelet and anticoagulant therapy combined with tirofiban and oxiracetam.The total effective rate,limb function recovery(as measured by Barthel index(BI)scores),neurological deficits(assessed by National Institutes of Health Stroke Scale(NIHSS)scores),cognitive impairment(evaluated using mini-mental state examination(MMSE)and Montreal Cognitive Assessment(MoCA)scores),peripheral blood complement C3 and C4 levels,and the incidence of adverse reactions between the control group and the observation group after 14 days of treatment.Results The total effective rate was significantly higher in the observation group than in the control group(98.25%(56/57)vs 85.96%(49/57),χ2=4.343,P<0.05).After treatment,the BI score was significantly higher in the observation group than in the control group((50.19±3.48)vs(47.65±2.72),t=4.342,P<0.001),while the NIHSS score was lower in the observation group than in the control group((5.28±1.50)vs(6.95±1.66),t=5.635,P<0.001).Additionally,the MMSE score((20.70±5.58)vs(18.44±4.20),t=2.443,P=0.016)and MoCA score((18.95±5.17)vs(16.07±5.00),t=3.020,P=0.003)were significantly better in the observation group compared to the control group.The levels of complement C3((2.30±0.51)mg/L vs(1.90±0.56)mg/L,t=3.987,P<0.001)and C4((1.84±0.22)mg/L vs(1.47±0.24)mg/L,t=8.580,P<0.001)in the observation group were also higher than those in the control group.There was no sig-nificant difference in the incidence of adverse reactions between the two groups(12.28%(7/57)vs 15.79%(9/57),χ2=0.291,P=0.590).Conclusion The combination of tirofiban and oxiracetam significantly improves neurological deficits,cognitive dys-function,and markedly elevates complement C3/C4 levels in PIS patients,with good safety profile.
